目的:探讨经脐单孔腹腔镜手术治疗肝脏恶性肿瘤的可行性及临床应用前景.方法:回顾分析2010年3月至2011年5月为2例患者施行单孔腹腔镜右肝恶性肿瘤切除术的手术方法及临床效果.结果:2例均成功完成单孔手术,无一例中转开腹或增加切口.手术时间分别为75 min和89 min,术中出血60 ml和50 ml,切除肿瘤直径分别为3.5 cm与3.6 cm,术后病理检查结果与术前诊断一致,肿块切缘未见癌细胞浸润.术后无感染、腹腔积液及胆漏等并发症发生,术后1周内出院.结论:单孔腹腔镜右肝恶性肿瘤切除术安全、可行,但术者需同时具备单孔腹腔镜手术及传统腹腔镜肝脏手术的操作经验.相信随着器械的改进,此项技术有望进一步推广应用.%Objective:To explore the feasibility and clinical value of transumbilical single incision laparoscopic hepatectomy for liver cancer. Methods: From March 2010 to May 2011 two patients with right liver caner have been performed by transumbilical single incision laparoscopic hepatectomy. The clinica data of the two cases were analyzed retrospectively. Results: Two cases have been performed successfully by transumbilical single incision laparoscopic hepatectomy without conversion to laparotomy or conventional laparoscopic surgery. The operative time was 75 and 89 min and blood loss was 60 and 50 ml respectively. Diameter of the resected tumor was 3.5 and 3. 6 cm. HCC was confirmed histoiogically in both specimens. No complication occurred and both patients were discharged in a week. Conclusions:Transumbilical single incision laparoscopical hepatectomy is a safe and feasible procedure for liver cancer provided that the operator has the experience both in single incision laparoscopic surgery and traditional laparoscopic liver surgery. With the improvement of equipment, the practical procedure is expected to get the further application.
