术中超声在内镜微创保胆取石术中的应用价值

摘要

Objective:To explore the clinical value of intraoperative ultrasound applied in endoscopic gallbladder preserving cholecystolithotomy.Methods:A retrospective analysis was made on the clinical data of 212 patients who suffered from gallstones and underwent intraoperative ultrasound-assisted endoscopic gallbladder preserving cholecystolithotomy from Jan.2014 to Mar.2015.Results:In the 212 patietns,200 patients were successfully performed endoscopic gallbladder preserving cholecystolithotomy,11 patients were converted to laparoscopic cholecystectomy and the rest 1 underwent open radical cholecystectomy.In 9 of these 200 operations,intraoperative ultrasound enabled surgeons to find and remove calculi which had not been detected before operation.No cholecystolithiasis recurrence was found by B-ultrasonography during the follow-up of more than 1 year.Conclusions:The application of intraoperative ultrasound in endoscopic gallbladder preserving cholecystolithotomy,which is a complementary technology for choledochoscopy and laparoscopy,can provide more effective and reliable information for surgeons,then surgeons can make operational decisions more reasonably,remove stones more thoroughly and promote the quality of surgeries.%目的:探讨B超在内镜微创保胆取石术中的临床应用价值.方法:回顾分析2014年1月至2015年3月采用B超术中辅助行内镜微创保胆取石术的临床资料.结果:212例患者中,200例最终成功施行内镜微创保胆取石术,11例改行腹腔镜胆囊切除术,1例开腹行胆囊癌根治术.保胆的200例患者中,术中B超探查发现残余结石9例,反复取石直至取尽.术后随访1年以上,B超检查提示无一例胆囊结石复发.结论:内镜微创保胆取石术中应用普通B超能与胆道镜、腹腔镜优势互补,提供更丰富、更确切的临床信息,优化手术决策,利于取尽结石,提高手术质量.
