悬吊式腹腔镜及气腹腹腔镜手术对卵巢囊肿剥除术患者卵巢功能、炎症反应及应激激素的影响

摘要

目的:探讨悬吊式腹腔镜与气腹腹腔镜手术对卵巢囊肿剥除术患者卵巢功能、炎症反应及应激激素的影响.方法:回顾性分析接受腹腔镜卵巢囊肿剥除术患者作为研究对象,依据患者接受的手术方式分为对照组和治疗组,比较术前及术后24 b炎症因子与应激激素水平及术前,术后3个月月经来潮第3 d卵巢功能.结果:术前两组间 TNF-α、CRP、COR、NE、E2、LH及FSH 水平差异无统计学意义(P>0.05);术后24 b治疗组TNF-α、CRP、COR及NE水平均显著低于对照组,术后3个月治疗组E2水平显著高于对照组,L H及FS H水平显著低于对照组,差异有统计学意义(P<0.05).结论:与气腹腹腔镜手术相比,悬吊式腹腔镜手术可有效减轻卵巢囊肿剥除术患者的炎症反应,降低应激激素水平,改善卵巢功能,具有一定临床价值.%Objective:To investigate tbe effects of suspension laparoscopy and pneumoperitoneum laparoscopic surgery on inflammatory response,stress bormone and ovarian function in patients witb ovarian cystectomy.Methods:Retrospective a-nalysis of tbe data of laparoscopic ovarian cyst denudation in our bospital from July 2015 to August 2017,and tbey were were divided into tbe control group and tbe treatment group according to tbe surgical metbods tbey accepted.Tbe levels of inflamma-tory factors and stress bormone preoperative and postoperative 24 b,and ovarian function of tbe menstrual period 3rd day pre-operative and postoperative 3 montbs of tbe two groups were compared.Results:Tbere was no significant difference in tbe lev-els of TNF-α,CRP,COR,NE,E2,LH and FSH between tbe two groups before tbe operation(P>0.05).Postoperative 24 b,tbe levels of TNF-α,CRP,COR and NE in tbe treatment group were significantly lower tban tbose in tbe control group, and tbe level of E2 in tbe treatment group was bigber tban tbat in control group,tbe levels of LH and FSH in tbe treatment group were lower tban tbose in control group postoperative 3 montbs,tbe difference were statistically significant(P<0.05). Conclusion:Compared witb pneumoperitoneum laparoscopic surgery,suspended laparoscopic surgery can effectively reduce tbe inflammatory response,reduce tbe level of stress bormone and improve ovarian function in patients undergoing ovarian cystec-tomy,wbicb bas a certain clinical value.
