高频超声结合萤火虫技术对痛风性关节炎的诊断价值

         

摘要

目的:探讨高频超声(HFUS)结合萤火虫技术对痛风性关节炎(GA)的诊断价值.方法:对47例GA患者(235个关节)进行HFUS结合萤火虫技术及双源CT(DECT)检查,分析比较两种检查方式的影像学特征、微结晶肿痛关节数目及两者对GA的诊断效能.结果:对于双轨征、滑膜增厚、滑膜炎及腘窝囊肿,HFUS结合萤火虫技术与DECT检出率比较差异有统计学意义(P<0.05);对于骨侵蚀和关节积液,HFUS结合萤火虫技术与DECT检出率比较差异无统计学意义(P>0.05);HFUS结合萤火虫技术检测出的微结晶肿痛关节数目高于DECT(P<0.05).两种检查方法的受试者工作特征(ROC)曲线下面积比较,差异无统计学意义(P>0.05).结论:HFUS结合萤火虫技术对于GA的诊断具有重要价值.%Objective:To study the value of high frequency ultrasound (HFUS) combined with MicroPure for gouty arthritis (GA).Methods:HFUS combined with MicroPure and dual source CT (DECT) were respectively examined in 47 cases (235 joints) with GA.The imaging features,the number of microcalcifications and the diagnostic value for GA were analyzed and compared between the two methods.Results:There was significant difference in the detection rate of the "double contour sign",synovial hypertrophy,synovitis and popliteal cyst between the two methods (P<0.05),but no significant difference was found in the bony erosion and joint effusion (P>0.05).The number of microcalcifications detected by HFUS combined with MicroPure was higher,compared with DECT examination (P<0.05).There was no significant difference in the area under the ROC curve between the two methods (P>0.05).Conclusion:HFUS with Micro-Pure had an important value for the diagnosis of GA.
