目的:探讨重度异常脑电图的脑梗死患者溶栓后复查脑电图的表现与溶栓效果及远期预后的相关性.方法:对75例重度异常脑电图的脑梗死患者进行静注纤维蛋白原酶激活剂(rt-PA)溶栓治疗,于溶栓前及溶栓后2h均行脑电图检测并对其结果进行Synek分级,比较溶栓成功组与不成功组Synek分级的差异;所有患者于发病后90 d用美国国立卫生研究院卒中量表(NIHS)评分及改良RANKIN量表(mRS)评分预后,采用Spearman相关性分析判断溶栓后脑电图Synek分级与上述评分的相关性.结果:溶栓成功组与不成功组的溶栓后脑电图Synek分级差异有统计学意义,U=415,P=0.07;重度异常脑电图的脑梗死患者溶栓后脑电图Synek分级与90 d后美国国立卫生研究院卒中量表(NIHS)评分及mRS评分均呈正相关,分别为r=0.868,r=0.886,P<0.05.结论:重度异常脑电图的脑梗死患者溶栓后脑电图表现与溶栓成功与否有关,溶栓后脑电图Synek分级与90 d NIHS评分及mRS评分均呈正相关.%Objective:To explore the correlation between severe abnormal EEG after thrombolytic therapy and thrombolytic effect and long-term outcome in acute ischemic stroke.Methods:Seventy-five patients with acute ischemic stroke were treated with intravenous recombinant tissue plasminogen activator (rt-PA).EEG data were obtained before thrombolytic therapy and after thrombolytic therapy 2 hours later.The difference of Synek classification between successful and unsuccessful groups after thrombolytic therapy was compared.The outcome was evaluated through 90-day NIHSS and Modified Rankin Scale (MRS).Spearman correlation analysis was performed on EEG after thrombolytic therapy and prognosis.Results:There was statistically significant in synek classification between successful and unsuccessful groups after thrombolytic therapy (U=415,P=0.07).Severe abnormal EEG after thrombolytic therapy was positively correlated with 90-day NIHSS and MRS(r=0.868,r=0.886,all P<0.01).Conclusion:Severe abnormal EEG after thrombolytic therapy is positively correlated with 90-day NIHSS and MRS.
