高危型HPV－DNA负荷量对早期宫颈癌患者生存状况的影响

摘要

目的：探讨高危型HPV-DNA负荷量对早期宫颈癌患者生存状况的影响。方法回顾性分析HPV-DNA呈阳性的患者共1421例(其中86例患者检测为早期宫颈癌)患者的临床资料，根据患者的( RLU/CO)比值大小分成两组， RLU/CO比值大于100的患者分为高病毒负荷量组，本组患者共41例，反之，低病毒负荷量组，本组患者共45例，比较两组患者的3年生存率，复发率，以及应用Cox回归进行多因素分析确定影响早期宫颈癌的独立危险因素。结果两组患者的平均年龄、临床FIGO、病理类型分布、病理风险因素的分布在两组患者之间的分布差异均无统计学意义，两组患者1年、2年、3年内生存率的差异均无统计学差异，病理类型为以非鳞状癌及淋巴结转移是早期宫颈癌患者复发的独立性危险因素。结论病理类型为非鳞状癌以及淋巴结转移这两个因素是早期宫颈癌患者复发的独立性危险因素，且HPV-DNA负荷量的高低对早期宫颈癌患者生存状况无明显的影响。%Objective To investigate the effect of high risk HPV-DNA load on survival in patients with early cer-vical cancer. Methods The clinical data of 1412 patients With HPV-DNA posieive was retrospectively analgzed,in which there have 86 patients wich early cervical cancer.According to the Value of (KLU/CO) ratio,the patients were divided into two groups. The ( RLU/CO ) ratio greater than 100 of the patients were divided into high viral load group, this group of patients with a total of 41 cases and, conversely, low virus load group, this group of patients with a total of 45 cases. The 3 years survival rate, recurrence rate, and the application of Cox regression in the two groups were used for multivariate analysis to determine the independent risk factors of early cervical cancer. Results The average age of the patients, clinical features, pathological types of FIGO distribution, the distribution of pathological risk factors between patients in the two groups were not statistically significant, the difference was not statistically sig-nificant in two groups of patients was 1 years, 2 years, 3 years survival rate, pathological type with non squamous carcinoma and lymph node metastasis are independent risk factors of early recurrence in patients with cervical cancer. Conclusion Pathological type was non squamous carcinoma and lymph node metastasis in patients with these two fac-tors are independent risk factors of patients with recurrent cervical cancer early, and HPV-DNA loading level on sur-vival in patients with early stage cervical cancer has not obvious effect.
