多层螺旋CT在原发性小肠恶性淋巴瘤诊断中的应用价值

摘要

目的 评价多层螺旋CT在小肠恶性淋巴瘤诊断中的价值.方法 回顾性分析41例小肠恶性淋巴瘤的CT检查资料,全部病例经手术或活检病理证实,均为非霍奇金淋巴瘤.结果 位于回肠28例,空肠11例,十二指肠1例,多发1例.41例患者CT均表现为不同程度的肠壁不规则增厚或腹腔肿块,其中表现为肠壁增厚19例,动脉瘤样扩张14例,肿块7例;累及多节段肠管的1例.结论 多层螺旋CT扫描在小肠恶性淋巴瘤的诊疗中,具有独特的优越性;在显示肠壁增厚的程度、形态、范围,浆膜面的改变,肠外器官的侵犯,淋巴结增大等方面能提供更详细的资料,有助于肿瘤的定位、诊断及分期.%Objective To assess the value of multislice CT scan in the diagnosis of primary small intestinal lymphoma.Methods Forty - one patients with primary small intestinal lymphoma underwent multi - slice CT dynamic enhancement scans, and axial images and reconstructive images were retrospectively analyzed.All patients were confirmed as non - Hodgkin' s lymphoma surgically or pathologically.Results Of 41 cases of primary small intestinal lymphoma,lesions were located in ileum(28 cases) ,jejunum (11 cases)and duodenum( 1 case)respectively,and there was one multifocal case.All 41 cases were observed with irregular intestinal wall thickening or celiac tumor to varying degrees,including 19 cases of intestinal wall thickening, 14 cases of aneurysm -like expansion ,7 cases of focal tumor, and one case of multiple segments of small intestine involved.Conclusion Multi - slice spiral CT scan has distinctive superiority in diagnosis of primary malignant lymphoma of small intestine.It provides more detailed data of the extent, shape, range of intestinal wall thickening, change in serous membrane, invasion of extraintestinal organs, enlargement of lymph nodes and so on, which helps the localization, diagnosis and staging of the lymphoma.
