新诊断2型糖尿病患者血清瘦素、脂联素及两者比值与颈动脉内膜中层厚度间的关系

摘要

[Objective]To explore the relationship of carotid intima-media thickness(CIMT) with serum levels of leptin and adiponectin and the ratio of leptin to adiponectin in newly diagnosed type 2 diabetic (T2DM ) patients .[Methods]CIMT of 58 newly diagnosed T2DM patients and 30 normal controls was measured by B-type ultrasound . ELISA was used to detect serum levels of leptin and adiponectin .The ratio of leptin to adiponectin was calculated .[Results]Compared with normal control group ,serum level of leptin ,the ratio of leptin to adiponectin and CIMT in newly diagnosed T2DM group were significantly increased ( P<0 .05 ,P<0 .01 and P<0 .01 ,respectively) ,but se-rum level of adiponectin was decreased( P<0 .01) .CIMT in T2DM group was positively correlated with age ,systol-ic blood pressure(SBP) ,body mass index (BMI) ,hip circumference ,low density lipoprotein-cholesterol(LDL-C) , leptin and the ratio of leptin to adiponectin significantly ( P <0 .01 ,P <0 .05) ,but negatively correlated with adi-ponectin .In newly diagnosed T2DM group ,CIMT was independently correlated with leptin ,adiponectin and the ratio of leptin to adiponectin( P<0 .05) .[Conclusion]The ratio of leptin to adiponectin may be a better clinical marker of predicting the early atherosclerosis in T 2DM patients .%[目的]探讨新诊断的2型糖尿病（T2DM ）患者血清瘦素、脂联素水平及瘦素/脂联素和颈动脉内膜中层厚度（CIM T ）的关系。[方法]以B型超声测定58名新诊断T2DM患者及30名正常对照者的CIM T ，以ELISA法测定其血清瘦素、脂联素水平，并计算瘦素/脂联素。[结论]①新诊断的 T2DM 组血清瘦素水平、瘦素/脂联素、CIM T较正常对照组显著升高而脂联素水平降低（均 P＜0．01）；②T2DM组CIM T与年龄、收缩压（SBP）、体质量指数（BMI）、臀围、低密度脂蛋白胆固醇（LDL-C）、瘦素、瘦素/脂联素呈显著正相关（ P＜0．01，P ＜0．05），而与脂联素呈负相关（ P ＜0．01）。③在新诊断的 T2DM 组，瘦素、脂联素、瘦素/脂联素与CIM T独立相关（ P ＜0．05）。[结论]瘦素/脂联素可能是预测T2DM 患者早期动脉粥样硬化程度更好的指标。
