肺表面活性物质联合CPAP治疗对新生儿呼吸窘迫综合征血气指标的影响

         

摘要

目的 研究肺表面活性物质联合CPAP对呼吸窘迫综合征新生儿血气指标的影响.方法 从2010年2月到2014年2月,于我院共有74例新生儿病患被诊断为呼吸窘迫综合征.以数字法随机分成治疗组(37例)和对照组(37例).其中治疗组给予肺表面活性物质联合持续气道正压通气(CPAP)进行治疗,对照组仅给予持续气道正压通气治疗,观察两组治疗疗效,以及两组患儿血气指标改善情况.结果 治疗组显效占比48.65%(18/37),总有效率为94.59%(35/37),均显著高于对照组的24.32%(9/37),75.68%(28/37);治疗后治疗组PH值以及P(CO2)/mmHg和P(O2)/mmHg改善程度均显著高于对照组;治疗后治疗组患儿PEEP/cmH2 O与FiO2/%均显著低于对照组;治疗前两组患儿OI指数差异无统计学意义,治疗后治疗组显著高于对照组,上述差异均有统计学意义(均P<0.05).结论 肺表面活性物质联合CPAP治疗NRDS疗效显著,且能显著改善患儿血气指标,值得推广.%Objective To study the effect of pulmonary surfactant combined with CPAP on blood gas indexes of neonatal respiratory distress syndrome. Methods From February 2010 to February 2014, a total of 74 neonatal disease patients were diagnosed as respiratory distress syndrome. They were randomly divided into the treatment group (37 cases) and the control group (37 cases). The treatment group was given pulmonary surfactant combined with continuous airway positive pressure ( CPAP) treatment, and the control group only received continuous positive airway pressure ventilation. The therapeutic effect and blood gas analysis indexes of the two groups were compared. Results The markedly effective rate was 48. 65% (18/37), and the total effective rate was 94. 59% (35/37) in the treatment group, which were significantly higher than those in the control group [24. 32% (9/37), 75. 68% (28/37)].After treatment, the improvement of PH value and P (CO2 )/mmHg and P (O2 )/mmHg was significantly higher inrnthe treatment group than in the control group. The levels of PEEP/ cm H2O and FiO2/% were significantly lower in the treatment group than in the control group. There was no significant difference in OI index between the two groupsrnbefore treatment, and it showed statistical significance after treatment (P < 0. 05). Conclusion The combination pulmonary surfactant and CPAP has significant effect in treatment of NRDS, and it can significantly improve the bloodrngas indexes, which is worthy of wide promotion.
