超声引导射频消融术治疗原发性肝癌中期随访结果分析

摘要

Objective To investigate the clinical effect of ultrasound guided radiofrequency ablation in the treatment of primary liver cancer.Methods 280 patients with primary liver cancer were randomly divided into the study group and the control group ,140 cases in each group.The control group received surgery , and the study group received ultrasound guided radiofrequency ablation.The levels of immune function , serum phosphatidylinositol proteoglycan 3 ,carcinoembryonic antigen and alpha fetoprotein were compared between the two groups before and after treatment.The incidence of complications was compared between the two groups ,followed up for 12 months ,and the recurrence rates of the two groups were compared. Results The levels of serum CD3+,CD4+,CD4+/CD8+in the study group were significantly higher than those in the control group (P＜0.01) ,and the level of CD8+decreased significantly (P＜0.01) ,and the change was more significant than that of the control group (P＜0.01).After treatment ,the serum levels of phosphatidylinositol proteoglycan 3 ,carcinoembryonic antigen and alpha-fetoprotein in the two groups were significantly lower than those before treatment ( P＜ 0.05) ,and there was no significant difference between the two groups (P＞0.05).The incidence of complications in the study group was sig-nificantly lower than that in the control group (P＜0.01).There was no significant difference in the recur-rence rate between the two groups at 6 months and 12 months after treatment (P＞0.05).Conclusion Ultrasound-guided radiofrequency ablation can effectively reduce the serum levels of GPC 3 ,CEA and AFP in patients with primary hepatocellular carcinoma ,regulate the immune function of the body ,and has a low incidence of complications ,which is worthy of popularization and application.%探讨超声引导射频消融术治疗原发性肝癌的临床效果.方法 将280例原发性肝癌患者按随机数字表法分为研究组与对照组各140例.对照组给予外科手术,研究组给予超声引导射频消融术.比较治疗前后两组免疫功能水平、血清磷脂酰肌醇蛋白聚糖3 、癌胚抗原、甲胎蛋白水平,比较两组并发症发生率,随访12个月统计两组疾病复发率.结果 治疗后研究组血清CD3+、CD4+、CD4+/CD8+水平显著升高(P＜0 .01) ,CD8+水平显著降低( P＜0 .01 ) ,较对照组变化更显著(P＜0 .01).治疗后两组磷脂酰肌醇蛋白聚糖3 、癌胚抗原、甲胎蛋白水平均较治疗前显著降低(P＜0 .05) ,组间比较差异无统计学意义(P＞0 .05).研究组并发症发生率显著低于对照组(P＜0 .01) ,治疗后6个月及12个月两组疾病复发率比较差异无统计学意义(P＞0 .05) .结论 超声引导射频消融术治疗原发性肝癌可有效降低患者血清磷脂酰肌醇蛋白聚糖3 、癌胚抗原、甲胎蛋白水平,调节机体免疫功能,并发症发生率低,值得推广应用.
