单臂上肢外固定器治疗小儿肱骨近端骨折

摘要

Objective To explore the results of manipulative reduction and Stryker' s extemal fixator in the treatment of proximal humeral fractures in children. Methods 25 children with proximal humeral fractures were treated with manipulative reduction and Stryker' s extemal fixator. Results Follow-ups lasted from 6 to 15 months. The time of bone healing was 6 to 12 weeks. At the latest follow-up, 23 patients obtained the result ″very good" and 2 ″good″ in the Consant-Murley Score. Conclusions Manipulative reduction and Stryker' s extemal fixator,which is advantageous to fracture reduction and solid fixation , avoiding epiphyseal injury and early post operative mobilization of elbow and shoulder,is an effective method to treat proximal humeral fractures in children.%目的 探讨单臂上肢外固定器在小儿肱骨近端骨折治疗中的应用.方法 首都医科大学附属北京儿童医院应用闭合复位单臂上肢外固定器固定治疗25例儿童肱骨近端骨折,骨折愈合后拆除外固定器随访临床结果.结果 患儿年龄6～14岁,平均9.2岁;25例均获随访,随访时间6～15个月,平均10.3个月,骨折临床愈合时间6～12周.全部病例骨折愈合,外固定器拆除时间平均在术后52(40～89)d.Consant-Murley评分:优:23例;良:2例;差:0例.结论 闭合复位单臂上肢外固定器固定治疗小儿肱骨近端骨折操作简单,微创,骨折容易复位,固定满意,便于护理,可避免骨骺损伤和骨折再移位,利于术后早期功能锻炼,是治疗小儿肱骨近端骨折的较好方法.
