带旋髂深血管蒂骨瓣移位及骨髓移植治疗青壮年股骨颈头下型骨折的疗效观察

摘要

Objective to investigate the result of vascular pedicle bone , bone marrow transplantation and fixedby 3 holllow compression nail in adult infra-head femoral neck fracture.Methods From January 2009 ~January2012,16cases of adult infra-head femoral neck fracture were treated with vascular pedicle bone and bone marrow transplantation,meanwhile the fracture was fixed by 3 holllow compression nail.Results All cases are followed up for 2-5 years.15 cases healed entirely without femoral head necrosis at 5-10 months after operation. One cases had femoral head necrosis because of fracture was too Serious and weight loading too early. Conclusion Using vascular pedicle bone , bone marrow transplantation and fixed by 3 holllow compression nail can accelerate the healing of femoral neck fracture effectively,so it is a simple and effective way to prevent femoral head necrosis in the adults,and Avoids changing hip.%目的：探讨用带旋髂深血管蒂骨瓣移位+骨髓移植治疗青壮年股骨颈头下型骨折的临床疗效。方法从2009年1月～2012年1月，对16例股骨颈头下型骨折采用带旋髂深血管蒂骨瓣移位加骨髓移植治疗，同时应用3根空心加压螺纹钉内固定。结果术后随访2～5年，15例在术后5个月～10个月获得骨性愈合，未发现股骨头坏死。1例因骨折严重及过早弃拐负重，导致股骨头坏死。结论采用内固定及带血运皮质骨块植骨与植入红骨髓的方法，可以促进股骨颈头下型骨折早期愈合，防止股骨头坏死，避免换髋。
