可扩张管下单侧椎弓根螺钉固定加椎间融合治疗极外侧型腰椎间盘突出症

摘要

Objective To evaluate the clinical outcomes of unilateral pedicle screw fixation combined with lumbar interbody fusion for the treatment of far lateral lumbar disc herniation assisted by Mast Quadrant system.Methods A total of 27 patients with far lateral lumbar disc herniation were received operation of unilateral pedicle screw fixation combined with lumbar interbody fusion assisted by Mast Quadrant system.Results The operation time was 110 ～160 min,and blood loss was 150 ～350 ml.All patients were followed up for 14 ～56 months.Both the VAS and JOA scores at 3 months after operation or the final follow-up improved significantly,compared with those of preoperation (P ＜0.01),and there was no significant difference between postoperative 3 months scores and final follow-up(P ＞ 0.05).All the patients got favourable intervertebral fusion and there were no complications related to pedicle screw fixation such as internal fixation loosening,breakage and cage displacement.Conclusions Unilateral pedicle screw fixation combined with lumbar interbody fusion for the treatment of far lateral lumbar disc herniation assisted by Mast Quadrant system has a favourable clinical outcome.%目的 评价Mast Quadrant可扩张管下单侧椎弓根螺钉固定加椎间融合术治疗极外侧型腰椎间盘突出症的临床效果.方法 采用Mast Quadrant可扩张管下单侧椎弓根螺钉固定加椎间融合术治疗27例极外侧型腰椎间盘突出症患者.结果 手术时间110 ～ 160 min,术中出血量150～350 ml.患者均获得随访,时间14～ 56个月.疼痛VAS评分和JOA评分:术后3个月与末次随访均较术前明显改善,差异有统计学意义(P＜0.01),末次随访与术后3个月比较差异无统计学意义(P＞0.05).复查见椎间植骨融合良好,无内固定松动、断裂及融合器移位等情况发生.结论 Mast Quadrant可扩张管下单侧椎弓根螺钉固定加椎间融合术治疗极外侧型腰椎间盘突出症有良好的临床效果.
