目的 探讨采用弹性髓内钉的微创手术治疗大龄儿童股骨干骨折的临床意义.方法 回顾性分析2007年08月~2010年08月大龄儿童股骨干骨折23例的手术治疗效果,C型臂透视下行骨科牵引床上复位,采用股骨内、外侧髁上小切口,插入弹性髓内钉,部分不稳定性骨折术后行支具固定4周,观察手术时间、切口长度、术中失血、住院时间及骨折愈合时间等指标.结果 23例术后随访10个月至24个月,均在6周至12周内临床愈合,无任何并发症及后遗症,平均手术时间(45±15)min,平均手术切口(2±0.7) cm,术中失血(45±12) mL,住院时间(10±2)天,骨折愈合时间(9±0.8)周,优良率达96%.结论 弹性髓内钉治疗大龄儿童股骨干骨折是可行而有效的微创手术方法,具有较好的临床疗效.%Objective To evaluate the clinical treatment outcome of minimally invasive op-eration using elastic intramedullary nailing for femoral shaft fracture in older children. Methods 23 patients with displaced femoral shaft fractures were treated by elastic intramedullary nailing from August 2007 to August 2010. The operation duration, scar length, bleeding volume, length of stay, fracture healing time and post - operation complication were observed. Results All the pa-tients were followed up range from 10 to 24 months. The patients had attained bone healing with anatomic paraposition in the six weeks to 12 weeks period and without any complication or sequela. According to Flynn scoring system, there were 19 excellent, 3 good, 1 fair, and the excellent -good rate is 96% . Conclusion Elastic stable intramedullary nailing was effective and minimally in-vasive operation in the treatment of femoral fracture in older children.
