i-gel喉罩在婴幼儿眼科手术中的应用

摘要

Objective To study the efficacy of the laryngeal mask airway i-gel in children undergoing eye surgery.Methods Sixty non-hospitalized children were randomized into 2 groups:LMA group(group L)as control group,and LMA i-gel group(group Ⅰ) as observation group,30cases in each group.Children in both of the two groups were induced and maintained with inhalation combined with sevoflurane-02.The HR,SpO2,RR,ETCO2 were measured at different times during surgery and at the end of surgery.The success times of first attempt of both groups,and the complications (sore throat,odynophagia,hoarseness) 2 days after the surgery,were recorded.Resuits There were no significant differences in HR,SpO2,RR,ETCO2 at different times during surgery and at the end of surgery (P ＞ 0.05).Compared with group S,the intubation time in group Ⅰ was shorter,and the rate of sore throat and odynophagia was lower (P ＜ 0.05).Conclusion Both LMA and LMA I-gel cannot make obvious hemodynamic effect during operation.Compared with LMA,using LMA I-gel has less sore throat,shorter placement time and higher first attempt success rate and less after operation complications.%目的 观察i-gel喉罩在婴幼儿眼科手术中的临床应用效果.方法 选择门诊眼科短小手术婴儿60例,随机分为2组,每组30例观察组为i-gel喉罩组,对照组为普通喉罩组.麻醉诱导和维持均用七氟烷,记录麻醉诱导前T0、麻醉诱导后T1、置入喉罩时T2、置入喉罩3 minT3、置入喉罩10 min T4、手术结束拔除喉罩前T5、拔除喉罩时T6、拔除喉罩3minT7等时段的心率;记录置入喉罩时间和置入喉罩5 min T8、20 min T9、手术结束时T10、SpO2、PetCO2、Ppeak;记录喉罩一次置入成功次数和术后2d内患儿声音及咽部并发症(异常哭闹和哺乳异常哭闹)发生率及恶心呕吐发生率.结果 2组各时段血液动力学变化差异无统计学意义(P＞0.05),通气指标SPO2、PetCO2、Ppeak差异也无统计学意义(P＞0.05),但2组喉罩置入时间和手术后咽部不适及恶心呕吐发生率差异有统计学意义(P＜0.05).结论 i-gel喉罩与普通喉罩相比在通气指标和对患儿血液动力学影响相似,但置入更容易快捷,手术后的并发症更少.
