多层螺旋 CT 多平面重组技术在评估术前胃癌 TN 分期中的应用价值

摘要

Objective To analyze the value of multi planar reconstruction technique of multi-slice spiral CT in the diagnosis of TN staging of gastric cancer.Methods 45 gastric cancer pa-tients were conducted with preoperative abdominal multi-slice spiral CT scan,and multi planar re-construction (MPR)was used to analyze the CT data.The processed results were analyzed by 3 ra-diologists.Results Consistency of spiral CT and pathologic findings for T staging diagnosis of gas-tric cancer was 88.4%,while that was 70.1% for N staging diagnosis.MPR was better than the transverse image examination,and there were significant differences of accuracy for the T staging and N staging detection,sensitivity and specificity between two methods (P <0.05).Conclusion Multi-slice spiral CT examination is one of the best methods for preoperative TN staging of gastric cancer,and it is worthy of clinical application.%目的：分析多层螺旋 CT 多平面重组技术在胃癌 TN 分期诊断中的应用价值。方法45例胃癌患者术前行上腹部多层螺旋 CT 多期扫描，图像经多平面重组（MPR）技术处理后，由3名放射科医师进行分析。结果胃癌螺旋 CT 和病理结果对 T 分期诊断一致性为88．4％，对 N 分期诊断一致性为70．1％。MPR 检测结果明显优于横断位图像检查结果，二者对 T分期和 N 分期检测的准确率、敏感度和特异性比较，差异有统计学意义（P ＜0．05）。结论多层螺旋 CT 多平面重组技术是判断胃癌术前 TN 分期较理想的方法之一，值得临床推广应用。
