2型糖尿病患者糖化血红蛋白及氧化相关物质与肺功能的相关性研究

摘要

Objective To explore the correlation between peripheral blood glycosylated hemoglobin (HbA1c) with oxidative related substances and pulmonary function in patients of type 2 diatetes mellitus (T2DM).Methods HbA1 c,superoxide dismutase (SOD),glutathione peroxidase (GSH-Px),reactive oxygen species (ROS),malondialdehyde (MDA)and pulmonary function indexes such as vital capacity (VC),forced vital capacity (FVC),forced expiratory volume in 1 st second (FEV1),FEV1/FVC,diffusion capacity of carbon monoxide in lung (DLCO),DLCO/VA of 94 patients with T2DM (T2DM group) and 90 healthy people(control group) were tested.Calculate the percentage of actual measured value of lung function index(％ pred) and the correlations between pulmonary function with HbA1c and oxidative indexes were analyzed.Results VC％ pred,FVC％ pred,FEV1％ pred,FEV1/FVC％ pred,DLCO％ pred and DLCO/VA％ pred in T2DM group were lower than those in control group(P ＜ 0.05).Correlation analysis showed that in T2DM patients,the pulmonary function was positively correlated with SOD and GSH-Px levels while negatively correlated with ROS and MDA levels (P ＜ 0.05).And there was a negative correlation between HbA1c and pulmonary function (P ＜ 0.05).Conclusion Oxidative stress is involved in the process of diabetic lung injury and elevated blood sugar is a risk factor for pulmonary function injury.%目的 探讨2型糖尿病(T2DM)患者外周血糖化血红蛋白(HbA1c)含量及氧化相关物质与其肺功能的相关性.方法 检测94例T2DM患者(T2DM组)与90例健康者(对照组)外周血HbA1c、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、活性氧(ROS)、丙二醛(MDA)的水平及肺功能各项指标如肺活量(VC)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC、肺一氧化碳弥散量(DLCO)、肺单位体积一氧化碳弥散量(DLCO/VA),计算肺功能指标实际测量值占预计值的百分比(％ pred),并分析肺功能与HbA1c及氧化相关物质的相关性.结果 T2DM组VC％ pred、FVC％ pred、FEV1％ pred、FEV1/FVC％ pred、DLCO％ pred及DLCO/VA％ pred均低于对照组(P＜0.05).相关性分析结果显示,T2DM组患者的肺功能与SOD、GSH-Px水平呈正相关,与ROS、MDA水平呈负相关;HbA1c与肺功能呈负相关(P＜0.05).结论 氧化应激参与糖尿病肺损伤的过程,而血糖水平升高是肺功能损伤的危险因素.
