超声骨刀开窗术与正畸牵引联合治疗成人上颌唇侧埋伏阻生尖牙的临床疗效观察

摘要

目的：观察超声骨刀开窗术与正畸牵引联合治疗成人上颌唇侧埋伏阻生尖牙的临床效果，并与传统骨凿开窗法进行对比，以期为成人埋伏尖牙的临床治疗提供参考。方法选取上颌唇侧埋伏阻生尖牙成人患者30例，随机分为两组，超声骨刀开窗＋正畸牵引组15例，传统手术开窗＋正畸牵引组15例，比较两组患者的治疗效果。结果超声骨刀开窗＋正畸牵引组和传统手术开窗＋正畸牵引组的总有效率均为93.33％，两组比较无统计学差异（ P ﹥0.05）。超声骨刀开窗＋正畸牵引组舌侧扣的一次粘结成功率高于传统手术开窗＋正畸牵引组（ P <0.05）。超声骨刀开窗＋正畸牵引组的术后疼痛及面部肿胀程度均明显低于传统手术开窗＋正畸牵引组，且差异具有统计学意义（ P <0.05）。结论超声骨刀开窗术与正畸牵引联合治疗成人上颌埋伏阻生尖牙效果明确，术后反应小，值得临床推广应用。%Objective To study the clinical results of maxillary labially impacted canines treated by piezosurgery and orthodontic traction in adult patients and compare piezosurgery with traditional method( chisels)in the exposure of maxillary impacted canines. Methods Thirty a-dult patients with maxillary labially impacted canines were selected and divided randomly into 2 groups:15 patients in piezosurgery plus orthodontic traction group and another 15 patients in traditional method plus orthodontic traction group. The curative effects were compared between these two groups. Results The total curative rates of the 2 groups were same(93. 33%)( P ﹥0. 05). The successful attachment rate of lingual button to the impacted canine surface was higher in piezosurgery plus orthodontic traction group( P <0. 05). Postoperative pain and facial swollen were much less in piezosurgery plus orthodontic traction group than those in traditional method plus orthodontic traction group( P <0. 05). ConclusionCompared with traditional method,piezosurgery plus orthodontic traction method is better in the treatment of maxillary labially impacted canines in adult patient,which may produce more effective results and less postopertive complications.
