喉癌患者血清肿瘤标志物CYFRA21-1、CA724、CA199、CEA及SCCAg的检测及诊断价值

摘要

目的：探讨血清肿瘤标志物的检测对喉癌患者的诊疗价值。方法分析2010年1月至2014年1月接受手术治疗51例喉癌患者的临床资料，列为观察组；并纳入45例健康体检者作为对照组。观察比较两组患者血清肿瘤标志物细胞角蛋白19片段（CYFRA21-1）、糖类抗原72-4（CA72-4）、糖类抗原19-9（CA19-9）、癌胚抗原（CEA）及鳞状细胞癌相关抗原（ SCCAg）水平，并比观察组手术前各肿瘤标志物水平。采用ROC曲线分析血清肿瘤标志物对喉癌的诊断价值。结果观察组患者血清CYFRA21-1、CA724、CA199、CEA及SCCAg均高于对照组（ P <0.001）。观察组患者接受手术治疗后，其血清肿瘤标志物（ CYFRA21-1、CA724、CA199、CEA及SCCAg）呈现显著下降趋势，较手术前比较差异具有统计学意义（ P <0.001）。ROC曲线分析显示，当CYFRA21-1取截点值2.264 ng/ml时对喉癌的诊断敏感度为88.24%，特异度为93.33%，优于其它肿瘤标志物。结论血清肿瘤标志物CYFRA21-1、CA724、CA199、CEA及SC-CAg对喉癌具有一定诊断价值，其中CYFRA21-1的诊断价值最高。%Objective Our study was aimed to analyze the levels of CYFRA21-1、CA724、CA199、CEA and SCCAg in patients with la-ryngeal and diagnostic value. Methods Clinical data of patients with laryngeal cancer received operative treatment at our hospital from 2010 to 2014 were analyzed as Group A,51 cases. Healthy controls were enrolled as Group B,45 cases. Serum tumor marker cytokeratin 19 fragment (CYFRA21-1),carbohydrate antigen 72 -4(CA72 -4),carbohydrate antigen 19 -9(CA19 -9),carcinoembryonic antigen(CEA)and squamous cell carcinoma associated antigen( SCCAg)level were observed and compared in the two groups. Tumor marker levels of the observation group was compared before and after surgery. ROC curve analysis was used to analyze the diagnostic value of serum tumor markers for laryngeal cancer. Results Group A had higher levels of CYFRA21-1,CA724,CA199,CEA and SCCAg than these of Group B,the difference were sta-tistically significant( P <0. 001). After surgery,serum tumor marker reduced significantly( P <0. 001). The ROC analysis showed CYFRA21-1 had a diagnostic sensitivity of 88. 24% in the diagnosis of laryngeal cancer,a diagnostic sensitivity of 93. 33% with a cut-off value of 2. 264 ng/ml. Conclusion Detection of serum CYFRA21 -1、CA724、CA199、CEA and SCCAg levels can be applied to early diagnosis of laryngeal cancer.
