目的：探讨比较颈后路单开门与双开门椎管扩大成形术在治疗多节段脊髓型颈椎病的临床效果。方法选取2010年1月至2014年1月间进行治疗多节段脊髓压迫症患者120例作为研究对象，随机分成双开组和单开组。双开组采取双开门椎管扩大成形术治疗方式，而单开组采取单开门椎管扩大成形术治疗方式。治疗结束后，比较两组患者的临床治疗效果、出血量、手术时间以及并发症的发生率。结果单开组患者的临床疗效明显优于双开组；但单开组出血量较双开组多，差异有显著性（ P ﹤0.05）；两组手术持续时间差异无显著性（ P ﹥0.05）；在并发症的发生率方面，单开组优于双开组，差异有显著性（ P ﹤0.05）。结论颈椎后路单开门与双开门椎管扩大成形术对治疗多节段脊髓压迫症都具有较好的临床疗效，单开门椎管扩大成形术的临床效果更为显著；双开门椎管扩大成形术在手术时间、出血量方面更优，在临床中可以根据患者的具体情况采取不同的治疗措施。%Objective To explore and compare the clinical effect of cervical spondylosis cervical posterior single door with double door vertebral canal expanded plasty in the treatment of multiple segmental myelopathic type. Methods 120 cases of patients with spinal cord compres-sion syndrome were randomly divided into observation group and single door group. The clinical treatment,blood loss,operating time and the inci-dence of complications of the two groups patients were compared after treatment. Results The clinical therapeutic effect in single door group was better than double door group,but single door group had more blood loss than double door group. There was a significant difference( P ﹤0. 05). There was no significant difference in the duration of surgery. The incidence of complications of single open group is lower than that of double open group,the difference was significant( P ﹤0. 05). Conclusion Cervical posterior single door and double door vertebral both have better effect for the treatment of multiple segmental spinal cord compression syndrome. This method has better clinical curative effect and the clinical effect of sin-gle open canal expanded keratoplasty. The double open canal expanded better in operation time,blood loss,according to the specific condition of patients in the clinical different treatment measures.
