高通量血液透析对糖尿病肾病患者的微炎症与氧化应激相关因子的作用

         

摘要

目的:探讨高通量血液透析对糖尿病肾病患者的微炎症与氧化应激相关因子的作用。方法选择62例糖尿病肾病血液透析的患者,根据治疗方法分为高通量透析组(HFHD 组)和低通量透析组(LFHD 组),各31例。比较两组治疗前后超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSHPx),白介素(IL)-6、肿瘤坏死因子(TNF)-α、高敏 C-反应蛋白(hs - CRP)的水平变化。结果治疗前两组 SOD、GSHPx、IL -6、TNF -α、hs - CRP 无统计学差异。经过6个月治疗后,HFHD 组血清 SOD 和 GSHPx 水平显著上升,IL -6、TNF -α、hs - CRP 明显下降,与 LFHD 组相比差异显著( P ﹤0.05)。结论 HFHD 可以改善终末期糖尿病肾病血液透析患者的氧化应激及微炎症状态,对提高患者的生活质量,提升患者的生存率具有重要意义。%Objective To investigate the effects of high - flux hemodialysis on micro - inflammation and oxidative stress - related factors in patients with diabetic nephropathy. Methods 62 hemodialysis patients with diabetic nephropathy were divided into high - flux hemodialysis group(HFHD group)and low - flux hemodialysis group(LFHD group),with 31 cases each. Before and after treatment,the levels of superoxide dismutase(SOD),glutathione peroxidase(GSHPx),interleukin 6(IL - 6),tumor necrosis factor - α(TNF - α)and C - reactive protein(hs- CRP)were compared. Results Before treatment,SOD,GSHPx,IL - 6,TNF - α,hs - CRP had no significant difference. After six months of treatment,serum levels of SOD and GSHPx increased significantly and IL - 6,TNF - α,hs - CRP decreased significantly in HFHD group,with significant differences compared with LFHD group( P ﹤ 0. 05). Conclusion HFHD can improve oxidative stress and micro - inflammatory state in patients with end - stage renal disease,which can improve the quality of life of patients and improve patient survival rate.
