丙型肝炎肝硬化常规超声、弹性成像与肝纤维化4项之间的相关性分析

摘要

目的 对常规超声、弹性成像与肝纤维化4项在丙型肝炎肝硬化中的相关性进行分析.方法 选取于2013年10月到2015年10月收治的丙型肝炎肝硬化患者112例,对患者进行详细的身体检查,选定同期非丙型肝炎肝硬化患者112例为对照组.分别对两组患者进行常规超声检查,记录患者的肝脏的大小、形态、肝包膜、肝脏边缘、肝实质回声、结节的有无与大小、胆囊内壁的厚度、肝门静脉的内径和流速、患者的脾脏厚度及其腹水情况;对患者进行常规超声检查后,启动弹性成像功能对其进行弹性成像的实时定量检测分析,并记录患者的血清透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原(pcⅢ)、Ⅳ型胶原(CIV)4项肝纤维化指标,分析常规超声、弹性成像与肝纤维化4项指标的相关性.结果 两组患者在一般资料的比较中无显著差异(P >0.05).观察组患者肝脏变化率、肝包膜变化率、肝硬化程度、门静脉高压发生率均高于对照组患者,差异具有统计学意义(P 0.05). But the liver size elasticity quantitative values,liver capsule grading,grading echo liver,por-tal performance in the observed group were all higher than those of the control group(P <0.05). The experimental groups were quantified in liver tissue elasticity values of different Child-Pugh classification. In A grade,B grade,C grade,elastic quantitative value LFindex liver tissue were 2.14 ± 0.31,2.55 ± 0.35,2.95 ± 0.43,the difference was statistically significant(P <0.05). Elasticity quantitative value in liver size,liver capsule grading,grading echo liver,portal performance and interest UE regional liver tissue and serum hyaluronic acid,laminin,procollagen III,Ⅳ collagen were correlated(P <0.05). Conclusion Conventional ultrasound,elastography with liver fibrosis four in HCV patients are related.
