单孔胸腔镜下肺大泡切除治疗自发性气胸快速康复的研究

摘要

目的:通过对比分析单孔胸腔镜肺大泡切除术后恢复情况,探讨原发性自发性气胸(PSP)快速康复治疗的可行性及安全性.方法:将研究对象随机分为单孔胸腔镜组(n=22)及常规电视胸腔镜手术(VATS)组(n=28),对比分析2组手术时间、肺复张、术后疼痛、术后引流量情况等指标.结果:单孔胸腔镜组平均手术时间(23±5.2) min、术中平均出血(20±2.7) ml、术后平均引流量(337±13.2) ml,VAS疼痛评分(5.7±2.2)分,与VATS组比较差异有统计学意义(P＜0.05).2组肺复张情况、拔管时间及住院天数比较,差异无统计学意义.结论:单孔胸腔镜技术治疗效果佳,同时在降低术后切口疼痛或胸壁感觉异常,加快术后康复方面具有明显的优势,是微创胸外科新的方向.%Objective:To comparative analysis of the recovery of single hole thoracoscopic resection of lung bullae,and explore the feasibility and safety of the PSP rapid rehabilitation treatment.Method:The patients were randomly divided into single hole laparoscopic group and VATS group,VATS group of 28 cases,single hole laparoscopic group of 22 cases.The operation time,lung expansion,postoperative pain and postoperative drainage volume of the two groups were compared and analyzed.Result:The Bakri balloon group's average operation time was (23 ± 5.2) main,bleeding during operation was (20 ± 2.7) ml,postoperative drainage was (337 ± 13.2) ml,VAS pain score was (5.7±2.2),compared with the VATS group,the difference had statistical significance.Compared with the VATS group,the lung expansion,the time of remove drainage tube and the hospital days in Bakri balloon group had no statistical significance.Conclusion:The singleport video-assisted thoracic surgery had high cure rate,and could reduce the pain of the incision or the abnormal sensation of the chest wall,accelerate postoperative rehabilitation.It would be a new direction of minimally invasive department of thoracic surgery.
