支气管哮喘儿童血清25-(OH)D3、IgE 、IL-17 、TGF-β1水平变化及意义

         

摘要

目的 探讨支气管哮喘儿童血清25 羟基维生素D3[25(OH)D3]、免疫球蛋白E( IgE) 、白介素-17(IL-17)及转化生长因子-β1(TGF-β1)水平变化及临床意义.方法 选取河北省邯郸市妇幼保健院儿科2016年1月至2016年8月收治的喘息性哮喘患儿60例(哮喘组),依据哮喘发作病情分为轻度和中重度哮喘发作患儿各30例,设立同期河北省邯郸市妇幼保健院健康体检儿童20例为对照组;采集各组儿童静脉血2 mL,分离血清,采用酶联免疫吸附法(ELISA法)测定各组血清25(OH)D3、IgE 、IL-17及TGF-β1水平,并对比不同病情患儿25(OH)D3、IgE、IL-17及TGF-β1水平变化.结果 哮喘组患儿血清25(OH)D3水平低于对照组(P<0.05),血清IgE 、IL-17及TGF-β1水平高于对照组(P<0.05);轻度哮喘组患儿血清25(OH)D3水平高于中重度哮喘组(P<0.05),血清IgE 、IL-17及TGF-β1水平低于中重度哮喘患儿组(P<0.05);相关性分析结果表明,对照组25(OH)D3水平与 IgE 、IL-17、TGF-β1无相关性(P>0.05);哮喘组患儿25(OH)D3水平与IgE 呈负相关(r=-0.738,P<0.05),血清IL-17与TGF-β1 呈正相关(r=0.877,P<0.05).结论 25(OH)D3、IgE、IL-17和TGF-β1在哮喘患儿发作中起重要作用,哮喘患儿血清25(OH)D3水平下降,IgE、IL-17、TGF-β1水平上升,多提示病情加重,临床应注意防治.%Objective To explore the level changes of 25-(OH)D3, IgE, IL-17 and TGF-β1 in children with bronchial asthma and their clinical significance. Methods A total of 60 children with bronchial asthma who were admitted into Handan Maternity and Child Care Centers from January of 2016 to August of 2016 were included in the study and classified into the asthma group which was further divided into the mild asthma group and the moderate or severe asthma group according to the condition of asthma attack, and each group consisted of 30 cases. Another 20 healthy children who came for physical examinations were selected and classified into the control group. The volume of 2mL venous blood in each group was collected and the serum was separated. ELISA was used to detect the levels of serum 25-(OH)D3, IgE, IL-17 and TGF-β1. The level changes of 25-(OH)D3, IgE, IL-17, and TGF-β1 in those children were compared. Results The level of serum 25-(OH)D3 in the asthma group was significantly lower than that in the control group (P<0.05), while the levels of serum IgE, IL-17 and TGF-β1 were significantly higher than those in the control group (P<0.05). The level of serum 25-(OH)D3 in the mild asthma group was significantly higher than that in the moderate or severe asthma groups (P<0.05), while the levels of serum IgE, IL-17, and TGF-β1 were significantly lower than those in the moderate or severe asthma group (P<0.05). According to the results of the correlation analysis, the level of serum 25-(OH)D3 was not correlated with the levels of IgE, IL-17 and TGF-β1 respectively in the control group (P>0.05), while the level of serum 25-(OH)D3 was negatively correlated with that of IgE (r=-0.738,P<0.05) and level of serum IL-17 was positively correlated with that of TGF-β1 (r=0.877,P<0.05) in the asthma group. Conclusion The levels of serum 25-(OH)D3, IgE, IL-17 and TGF-β1 play an important role in the asthma attack. The reduced level of serum 25-(OH)D3 and the elevated levels of IgE, IL-17 and TGF-β1 suggests mostly that the condition is aggravated. Therefore, much attention should be paid to those indexes for the clinical prevention and treatment.
