目的:观察多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者白细胞介素-18(IL-18)和单核细胞趋化蛋白-1(MCP-1)水平变化,探讨IL-18、MCP-1在PCOS中的异常情况与胰岛素抵抗(IR)和肥胖的关系.方法:选择PCOS患者(PCOS组)47例,正常对照(对照组)29名,根据PCOS患者的体质量指数(BMI)和胰岛素抵抗指数(HOMA-IR)分成肥胖组30例和非肥胖组17例.再根据PCOS患者HOMA-IR分为IR组18例和非IR组29例.分别比较各组IL-18和MCP-1的变化,并对PCOS患者IL-18、MCP-1与BMI、腰臀比(WHR)、HOMA-IR进行相关性分析.结果:PCOS组IL-18、MCP-1均明显高于对照组(P<0.01).IL-18与BMI、HOMA-IR,MCP-1与BMI、WHR、HOMA-IR均呈正相关关系(P<0.01).结论:PCOS患者IL-18和MCP-1水平明显升高,与IR、肥胖有关.%Objective: To observe the levels of interleukin-18( IL-18) and monocyte chemoattractantprotein-1 ( MCP-1 ) in polycystic ovary syndrome(PCOS) patients and to explore that the concentrations of IL-18 and MCP-1 were abnormal in PCOS,and that these cytokines were relevant to insulin resistance or obesity. Methods: Forty-seven PCOS patients( PCOS group) and 29 healthy volunteers (control group) were included in the study. The PCOS patients were divided into two sub groups according to the body mass index ( BMI) and homeostasis model assessment ( HOMA-IR) . According to BMI ,30 patients were included in the obese group and 17 in non-obese group;according to Homa-IR,18 patients were included in the insulin resistant group and 29 in non-insulin resistant group. The IL-18 and MCP-1 were compared in each group. The correlation between IL-18, MCP-1 and BMI, waist-hip ratio(WHR) , HOMA-IR were also analyzed in the PCOS patients. Results:The levels of IL-18 and MCP-1 in the PCOS IR group and the obese group were significantly higher than those in the non-insulin resistant PCOS group and the non-obese PCOS group (P <0. 01 ). IL-18 levels were positively correlated with BMI and HOMA-IR; MCP-1 levels were positively correlated with BMI, WHR and HOMA-IR ( P < 0. 01 ) . Conclusions: The levels of IL-18 and MCP-1 in PCOS patients increase significantly, which is correlated with IR and obesity.
