负压封闭吸引装置联合臀大肌上部肌皮瓣在修复骶尾部Ⅳ期压疮中的临床应用

摘要

目的:探讨负压封闭吸引装置( VSD)联合臀大肌上部肌皮瓣在修复Ⅳ期压疮中的疗效. 方法:对骶尾部Ⅳ期压疮创面采用一期扩创VSD技术,二期臀大肌上部肌皮瓣修复缺损创面13例资料进行分析,创面最小5. 5 cm × 7 cm,最大11 cm × 15 cm. 结果:13例肌皮瓣全部成活,2例皮瓣远端边缘部分坏死,经换药治愈. 随访6个月至3年,1例因家庭护理不当而复发,余12例无复发. 结论:VSD联合臀大肌上部肌皮瓣修复Ⅳ期压疮效果确切.%Objective:To investigate the effects of the vacuum sealing drainage ( VSD ) combined with upper gluteus maximus myocutaneous flap in the repair of the pressure ulcer with Ⅳ phase. Methods:The pressure ulcer with Ⅳ phase in sacral tail of 13 cases were treated with the primary phase debridement VSD technology combined with second upper gluteus maximus myocutaneous flap. The minimum and maximum wound were 5. 5 cm × 7 cm and 11 cm × 15 cm,respectively. Results:All 13 myocutaneous flaps survived,part far edge of myocutaneous flaps necrosis in 2 cases were cured. All patients were followed up for 6 months to 3 years,the recurrence in 1 case caused by family condition and unrecurrence in 12 cases were found. Conclusions:The effect of the VSD combined with upper gluteus maximus myocutaneous flap in the repair of pressure ulcer with Ⅳ phase is good.
