目的：探讨个案管理模式对慢性肾功能衰竭患者健康状况的影响。方法：将慢性肾功能衰竭患者110例随机分为对照组和个案管理组各55例。对照组按临床护理常规进行护理，出院后接受定期随访和返院复查，个案管理组由以个案管理师为核心的管理团队实施入院时管理、住院期间管理、出院前管理和出院后跟踪管理的个体化综合治疗护理，分别记录两组患者健康促进生活方式、肾功能、生活质量改善情况。结果：个案管理组患者的健康促进生活方式及肾功能明显优于对照组（ P <0.05），生活质量得分显著优于对照组（P <0.01）。结论：个案管理模式能促进患者健康生活方式，改善慢性肾功能衰竭患者的肾功能，提高其生活质量，减少并发症的发生。%Objective To investigate the case management model for chronic renal failure patients health. Method 110 patients with chronic renal failure were randomly divided into a control group and case management group 55 cases in each group. The control group in clinical care routine care after being discharged back to the hospital to receive regular follow - up and review,case management group was given management when implementing admission by the case management division as the core management team,hospital management,hos-pital management and front track management after discharge individualized comprehensive treatment and care,health - promoting lifes-tyles,renal function,quality of life improved of two groups were recorded. Results The management of patients with health - promoting lifes-tyle and renal function of case management group was significantly better than the control group(P < 0. 05),quality of life scores of case management group were significantly better than the control group(P < 0. 01). Conclusion Patient case management model can promote healthy lifestyles and improve renal function in chronic renal failure patients,improve their quality of life,reduce the incidence of complica-tions.
