目的：探讨和分析硬膜外阻滞复合全身麻醉在上腹部手术中的临床效果观察。方法：将310例上腹部手术患者随机分为观察组和对照组，每组155例。观察组采用硬膜外阻滞复合浅全身麻醉方法，对照组采取静吸复合全身麻醉的方法，将两组患者的麻醉镇痛效果、术后恢复情况及并发症情况进行分析和比较。结果：两组患者在全身麻醉诱导气管插管后心率、血压均显著下降，差异有统计学意义（P <0.05）；对照组患者在手术探查时，心率和血压有不同程度的升高，升高幅度显著高于观察组，差异有统计学意义（P <0.05）；观察组清醒时间为（7.0±1.6）min，对照组清醒时间为（18.7±6.2）min ，两组相比，差异有统计学意义（P <0.05）；观察组未出现恶心呕吐等并发症，对照组出现32例并发症，显著高于观察组，差异有统计学意义（ P <0.05）。结论：硬膜外阻滞复合浅全身麻醉在上腹部手术中的应用有效维持麻醉深度，达到良好稳定血流动力学的目的，大大降低了手术风险，值得临床推广使用。%Objective Discussion and analysis of epidural block in accordance with general anesthesia in the clinical effect of abdomi-nal surgery. Method 310 cases of abdominal surgery patients were randomly divided into observation group and control group,155 cases in each group. Observation group outside the shadow magic block composite light general anesthesia method,control group adopted the method of static absorption composite general anesthesia,two group of patients with anesthesia analgesic effect,postoperative recovery and complica-tions were analyzed and compared. Results Two groups of patients during general anesthesia induction of heart rate,blood pressure after en-dotracheal intubation were significantly decreased(P < 0. 05);Control patients in surgical exploration,heart rate and blood pressure rise to different extent,the increase was significantly higher than in observation group(P < 0. 05);Observation group waking hours(7. 0 ± 1. 6) minutes,control group waking hours(18. 7 ± 6. 2)minutes,two groups compared with significant difference(P < 0. 05);Nausea and vomi-ting complications such as observation group did not appear,the control group in 32 cases of complications,significantly higher than in ob-servation group(P < 0. 05). Conclusion Composite light general anesthesia epidural block in the application of abdominal surgery on effec-tively maintain anesthesia depth,achieve the goal of good stable hemodynamics,greatly reduce the risk of surgery,worth clinical promotion.
