超声引导经皮微波消融术联合米非司酮治疗子宫腺肌瘤的临床效果分析

摘要

目的：观察经皮微波消融术联合米非司酮治疗子宫腺肌瘤的临床疗效。方法：选取经超声、MRI、妇科专科检查诊断为子宫腺肌瘤患者21例行超声引导经皮微波消融术，术后给予米非司酮治疗3个月。结果：21例患者通过超声引导经皮微波消融术联合术后口服米非司酮治疗痛经症状明显改善，术前与治疗后痛经程度比较，差异具有统计学意义（P ＜0.05）；术前与治疗后3个月、6个月、12个月经周期和经期比较，差异无统计学意义（P ＞0.05）；术前与治疗后3个月、6个月、12个月经量比较，差异有统计学意义（P ＜0.05）；术前与治疗后3月、6月、12月血红蛋白比较差异具有统计学意义（P ＜0.05）；术前与治疗后3月、6月、12月雌二醇（E2）、促黄体生成素（LH）及卵泡刺激素（FSH）比较，差异无统计学意义（P ＞0.05）；术前与治疗后3月、6月、12月子宫体积比较，差异具有统计学意义（P ＜0.05），治疗后3月、6月、12月子宫体积小于术前。结论：超声引导经皮微波消融术联合米非司酮治疗子宫腺肌瘤能有效缓解患者痛经、月经量过多症状，改善月经过多所致的贫血状态，是一种有效、安全、微创的治疗方法。%Objective To observe clinical curative effect of the percutaneous microwave ablation in combination with mifepris-tone for the treatment of uterine adenomyoma.Method 21 patients with uterine adenomyoma diagnosed by ultrasound,MRI,special-ized gynaecology examination in our hospital underwent ultrasound guided percutaneous microwave ablation and took mifepristone in 3 months.Results 21 cases under the treatment of ultrasound -guided percutaneous microwave ablation in combination with mife-pristone,the dysmenorrhea symptoms improved postoperatively.The dysmenorrhea symptoms differed statistically between before and after treatment,the difference was statistically significant (P <0.05).There was no significant difference in menstrual cycle and pe-riod between before and after treatment in 3 months,6 months,12 months (P >0.05).The menstrual quantity differed statistically between before and after treatment,the difference was statistically significant (P <0.05).The hemoglobin differed statistically be-tween before and after treatment in 3 months,6 months,12 months,the difference was statistically significant (P <0.05).There was no significant difference in estradiol (E2 ),luteinizing hormone (LH),follicle -stimulating hormone (FSH)between before and af-ter treatment in 3 months,6 months,12 months (P >0.05).The uterine volume differed statistically between before and after treat-ment,the difference was statistically significant (P < 0.05 ).Uterine volume after treatment was lesser than be-fore.Conclusion Ultrasound -guided percutaneous microwave ablation in combination with mifepristone in the treatment of uterine adenomyoma can effectively alleviate symptoms of patients with dysmenorrhea,hypermenorrhea,improve the secondary anemia caused by menorrhagia,which is an effective,safe and minimally invasive treatment.
