自行调节式口腔矫治器对OSAHS患者的治疗效果

摘要

目的 探讨自行调节式口腔矫治器治疗阻塞性睡眠呼吸低通气暂停综合征(OSAHS)患者舌体及软腭的变化.方法 采用X线头影测量分别对32例OSAHS患者戴用矫治器治疗前、医师经验位和患者调节位的舌体及软腭的变化进行测量.结果 与治疗前比较,OSAHS患者戴用矫治器后,医师经验位和患者调节位睡眠呼吸暂停低通气指数分别降低了87.6%和93.1%,舌根后气道间隙分别增宽了19.56%和35.13%,软腭舌接触长度分别缩短了45.40%和52.39%.结论 自行调节式口腔矫治器于患者调节位时能更有效的减少上气道狭窄,减轻睡眠呼吸暂停.%Objective To investigate the changes of the tongue and soft palate in patients with obstructive sleep apnea hypoventilation syndrome(OSAHS) treated with the self-adjustable oral appliance. Methods Cephalometric measurements by X-ray analysis were carried out in 32 OSAHS patients to compare the changes of the tongue and soft palate before and after the self-adjustable oral appliance in a physician experience position (P1) and in a patient self-adjustable position ( P2 ),respectively. Results Compared to before, the obstructive sleep apnea hypoventilation index(OSAHI)was decreased by 87.6％ in P1 and 93.1％ in P2,the airway gap of behand the tongue was broadened by 19.56％ in P1 and 35.13％ in P2,and the length of touch area between the tongue and the soft palate. Conclusion To place the self-adjustable oral appliance in P2 is more effective than that in P1 in attenuating airway obstruction in OSAHS patients.
