2型糖尿病患者腹腔镜下Roux-en-Y胃旁路手术后的胰岛功能分析

摘要

目的 探讨2型糖尿病(T2DM)患者腹腔镜下Roux-en-Y胃旁路手术(LRYGB)后的胰岛功能.方法 评估行LRYGB的18例T2DM患者术前、术后6个月和术后1年的术后血糖水平、胰岛功能指标及代谢指标变化.结果 与术前比较,患者术后6个月和术后1年的FBG、餐后2 h血糖、HbA1c、BMI、LDL-C、TC、TG、腰臀比和HOMA-IR均降低(P<0.05),但术后6个月和术后1年上述指标比较差异无统计学意义(P>0.05).手术前后稳态模型β细胞功能指数和稳态模型胰岛素敏感指数等指标无统计学差异(P>0.05).结论 T2DM患者行LRYGB术后血糖下降的机制可能主要是由于其胰岛素抵抗状态的改善.%Objective To explore the islet function of patients with type 2 diabetes mellitus (T2DM) after laparoscopic Roux-en-Y gastric bypass surgery(LRYGB) .Methods The blood glucose levels ,islet function and metabolic changes were evaluated in 18 T2DM patients underwent LRYGB before surgery ,6 months and 1 year after surgery .Results Compared to before ,the levels of FBG ,2-h postload glucose ,HbA1c ,BMI ,LDL-C ,TC ,TG ,WHR and HOMA-IR were significantly lower in 6 months and 1 year after surgery (P<0 .05) ,which after surgery were not significantly different between those in 6 months and 1 year(P>0 .05) .Homeostasis model assessment of β-cell function index and insulin sensitivity index were not significantly changed before and after operation (P>0 .05) .Conclusion The mechanism of blood glucose decline after LRYGB in T 2DM patients is mainly due to the improvement of insulin resistance .
