调节免疫中成药与SLE感染关系研究

摘要

目的 选用调节免疫中成药(百令胶囊及六味地黄丸)联合免疫抑制治疗系统性红斑狼疮,为降低单纯免疫抑制治疗SLE伴发的高感染率作一探索.方法 病例选用符合诊断标准(采用美国风湿病学会[ACR]1997年推荐的分类标准)的系统性红斑狼疮患者,回顾性研究2008年1月1日至2011年12月31日伴有肾脏受累及肺间质性病变的SLE患者61例(均来自以上两医院规范治疗并定期随诊的患者),设计中西医结合治疗组(31例),西药对照组(30例).西药对照组采用糖皮质激素加环磷酰胺,中西医结合治疗组在对照组基础上加六味地黄丸及百令胶囊,通过治疗前后ESR,CRP,C3,C4,24 h尿蛋白,ANA,ds-DNA,血常规等变化.结果 整个疗程平均感染率,治疗组为31.5％,对照组为43.7％,治疗组感染率明显低于对照组.且治疗组总有效率(83.8％)显著高于对照组(73.3％).结论 调节免疫中成药(百令胶囊及六味地黄丸),可减轻免疫抑制剂引起的不良反应及并发症,从而提高疗效,预防和减少感染.%Objective To select and study the regulating immune medicines (bailing capsule and six taste glutinous rehmannia ball) combined with immunosuppressive therapy in systemic lupus erythematosus (SLE),to reduce simple immunosuppressive therapy in the treatment of SLE with high infection rates for exploration.Methods Case selection followed the diagnostic criteria (using American College of Rheumatology [ACR]1997 recommended classification criteria) in patients with SLE,retrospective study from January 1,2008 to December 31,2011 with renal involvement and pulmonary interstitial lesions in 61 patients with SLE (from more than two hospitals to standardize treatment and regular follow-up of patients),design combination of traditional Chinese and western medicine treatment group (31 cases),western medicine group (30 cases).The control group of western medicine used the sugar cortical hormone plus cyclophosphamide,combine with traditional Chinese and western medicine in treating group and control group based on the six taste glutinous rehmannia ball and Corbrin capsules by ESR,before and after the treatment,CRP,C3,C4,24h urine protein,ANA,ds-DNA,blood routine changes were all observed throughout the course of treatment.Results The average infection rate was 31.5％ in treatment group,43.7％ in the control group,treatment group was lower than that of control group.And the total effective rate of the treatment group (83.8％) was significantly higher than that in control group (73.3％).Conclusions The regulation of the immune medicines (bailing capsule and six taste glutinous rehmannia ball),can reduce the immune inhibitor induced adverse reactions and complications,thus enhance the curative effect,prevent and reduce the infection.
