经椎旁肌间隙入路伤椎内固定治疗无神经损伤胸腰椎骨折临床效果观察

摘要

Objective To discuss the clinical effects of treating thoracolumbar fractures without nerve damages via posterior pedicle fixation and vertebral bone graft through the gap of paraspinal muscles.Methods 600 patients of vertebral fractures were divided into the groups of observation and control.The systematic therapies of the pedicle screw were given to patients,through the gap of paraspinal muscles for the observation group,and through the traditional posterior approach for the control group.Then we observed the efficacy in both groups.Results The observation group was obviously less than the control group in terms of the blood loss during the operation,bed time and pain scores after the operation (P ＜ 0.05).The correction rate of both Cobb angle and vertebral collapse was comparable to one another (P ＞ 0.05).Conclusion Treating thoracolumbar fractures without nerve damages via posterior pedicle fixation and vertebral bone graft through the gap of paraspinal muscles,has the advantages of fewer traumas and less blood loss during the operation,and fast postoperative recovery,which is consistent with the modern minimally invasive concept.%目的 探讨经椎旁肌间隙入路伤椎内固定治疗无神经损伤胸腰椎骨折的临床疗效.方法 将600例胸腰椎骨折患者分为观察组和对照组,两组患者均给予椎弓根钉棒系统治疗,观察组采用经椎旁肌间隙入路,对照组采用传统后正中入路,观察两组患者治疗效果.结果 观察组患者术中出血量、术后卧床时间、术后疼痛评分明显低于对照组(P＜0.05);两组患者Cobb角纠正率及椎体塌陷纠正率比较差异无统计学意义(P＞0.05).结论 经椎旁肌间隙入路伤椎内固定治疗无神经损伤胸腰椎骨折,具有手术创伤小、术中出血量少、术后恢复快等优点,符合现代微创治疗理念.
