目的 探讨外侧锁定钛板与外侧锁定钛板加内后侧小支撑钛板内固定术对复杂胫骨平台骨折患者的治疗效果.方法 将201 1年1月至2013年12月我院收治的复杂胫骨平台骨折患者20例为临床研究对象,使用计算机随机的方法将患者分为两组,每组10例.采用前外侧与前外侧加后内侧入路,应用高尔夫锁定钛板同高尔夫锁定钛板+“T”形钛板内固定治疗复杂胫骨平台骨折,对照组患者使用单侧锁定钛板内固定,研究组患者使用双侧钛板内固定.结果 两组患者手术时间、术中出血量、膝关节活动范围、骨折愈合时间比较,差异无统计学意义;研究组疗效评价高于对照组,差异有统计学意义(P＜0.05).结论 使用外侧锁定钛板加内后侧小支撑钛板内固定术对复杂胫骨平台骨折(Ⅴ型、Ⅵ型)患者可以取得良好治疗效果,但单侧使用锁定钛板内固定术治疗Ⅴ型胫骨平台骨折患者也可取得较好的临床效果,同样疗效确切,值得应用.%Objective To investigate the therapeutic effect of lateral locking titanium plate and lateral locking titanium plate combined with the rear supporting of small titanium plate internal fixation in the treatment of patients with complex tibial plateau fracture.Methods 20 cases of complex tibial plateau fracture treated in our hospital from January 2011 to December 2013 were selected as the object of clinical study and divided into 2 groups with 10 cases in each group with the computer random method.All patients adopted anterolateral and anterolateral combined with posteromedial approach,by golf locking titanium plate and golf locking titanium plate + "T" shape titanium plate internal fixation for the treatment of complex tibial plateau fracture.Control group received unilateral locking titanium plate internal fixation,while study group received bilateral titanium plate internal fixation.Results There were no statistically significant differences in operation time,intraoperative bleeding volume,range of motion of knee joint,fracture healing time between two groups;the therapeutic effect of study group was better than that of control group,with statistically significant difference (P＜0.05).Conclusions Lateral locking titanium plate combined with the rear supporting of small titanium plate internal fixation for complex tibial plateau fracture (Ⅴ and Ⅵ type) patients can obtain good therapeutic effect;but unilateral locking titanium plate internal fixation in the treatment of patients with Ⅴ type tibial plateau fracture can also obtain good therapeutic effect,with the same exact curative effect,worthy of application.
