目的 探讨弥漫大B细胞淋巴瘤(diffuselargeB-celllymphoma,DLBCL)中CD5蛋白的表达情况,及其与患者病理特征和疗效的相关性.方法 选取2011年3月至2016年2月期间本院收治的DLBCL患者116例,采用免疫组化方法对CD5表达情况进行检测,根据检测结果将其分为阳性组和阴性组,对两组患者的临床病理特征及治疗效果进行回顾性分析.结果 CD5表达阳性同血清β2-MG水平之间并无明显相关性(P=0.377),而同侵犯部位(P=0.000)、病理分期(P=0.000)、性别(P=0.000)、年龄(P=0.000)及B症状(P=0.000)存在明显相关性;在阳性组23例患者中,经化疗共计1例CR,2例PR,总治疗有效率为13.04％;阴性组93例患者中,共计29例CR,36例PR,总治疗有效率为69.89％,阴性组患者治疗总有效率明显高于阳性组,差异有统计学意义(P＜0.05).结论 弥漫大B细胞淋巴瘤CD5阳性患者同阴性患者病理特征存在较大差异,且前者往往预后较差.%Objective To investigate the expression of CD5 protein in diffuse large B cell lymphoma (DLBCL) and to explore its correlation with pathologic features and curative effect.Methods 116 patients with DLBCL treated at our hospital from March,2011 to February,2016 were selected and detected by immunohistochemical method;and they divided into a positive group and a negative group according to the detection results.The clinical and pathological characteristics and therapeutic effect were retrospectively analyzed.Results There was no significant correlation between the positive expression of CD5 and the serum level of β2-MG (P=0.377),but was with the location of invasion (P =0.000),pathological stage (P =0.000),sex (P=0.000),age (P =0.000),and B symptoms (P =0.000).Among the 23 patients of the positive group,after chemotherapy,1 case got CR and 2 PR,with a total effective rate of 13.04％.Among the 93 patients of the negative group,29 cases got CR and 36 PR,with a total effective rate of 69.89％.The total effective rate was significantly higher in the negative group than in the positive group,with a statistical difference (P ＜ 0.05).Conclusion The pathological features in the patients with DLBCL positive in CD5 are quite different from those of the patients negative in CD5 and the positive ones has poorer prognosis.
