上海张江中老年人群的胃食管反流病患病率及其与生活习惯的关系

摘要

目的 了解上海张江地区中老年人的胃食管反流病(GERD)的患病状况,分析其与生活习惯的关系.方法 采用横断面研究,整群随机抽取张江10个社区共2007名中老年人进行问卷调查,分析该地区中老年人群的GERD患病率特点及其与生活习惯的关系.结果 反流性疾病问卷(RDQ)筛选的GERD患病率为4.9%,60岁以上老年人的GERD患病率总体上随年龄增长而升高,男、女性的GERD患病率差异无统计学差异.有些生活习惯与GERD关系密切,其中经常失眠(OR= 3.233,P<0.000)、经常服用催眠药(OR= 2.446,P<0.007)、进食过饱(OR= 2.127,P<0.003)、便秘(OR=1.795,P<0.013)、工作或情绪经常紧张(OR= 1.824,P<0.022)是GERD的危险因素,而经常参与麻将等棋牌活动(OR= 0.446,P<0.027)是GERD的保护性因素.结论 上海张江地区60岁以上老年人的GERD患病率较中年人明显升高,男、女性的GERD患病率无明显差异,进食过饱、经常失眠、工作或情绪经常紧张、便秘和经常服用催眠药物与GERD的发生相关.%Objective This paper aims to investigate the prevalence of gastroesophageal reflux disease(GERD)in the middle-aged and elderly people in Zhangjiang,Shanghai,and to analyze its relationship with living habits.Methods A cross-sectional study was conducted in which a total of 2 007 middle-aged and elderly people from 10 communities in Zhangjiang were randomly selected to conduct a questionnaire survey.Resuits The prevalence of GERD screened by the reflux disease questionnaire (RDQ)was 4.9%.The prevalence of GERD in people over the age of 60 generally increases with the age.There was no significant difference in the prevalence of GERD between male and female.Some living habits were closely related to GERD.Frequent insomnia(OR= 3.233,P<0.000),frequent use of hypnotics(OR= 2.446,P<0.007),overfeeding(OR= 2.127,P<0.003),constipation(OR=1.795,P<0.013),and work or emotional stress(OR= 1.824,P<0.022)were risk factors for GERD. However,frequent participation in mahjong and other chess and card games(OR= 0.446,P<0.027)is a protective factor for GERD.Conciusions The prevalence of GERD in people over the age of 60 in Zhangjiang,Shanghai is significantly higher than that in middle-aged people.There is no significant difference in the prevalence of GERD between male and female.Overfeeding,frequent insomnia,work or emotional stress,constipation,and frequent use of hypnotics are related to the occurrence of GERD.
