抗病毒治疗时间对AIDS患者生命质量的影响研究

摘要

目的 探讨接受高效抗反转录病毒治疗时间的长短对AIDS患者生命质量的影响.方法 采取典型抽样的方法,选取1个省级、2个州级、5个县级抗病毒治疗医院为研究现场,选择2009年4月前在这8个研究现场入组并接受抗病毒治疗的AIDS患者作为研究对象,使用中文版SF-36量表和自编基本情况调查表对其进行横断面调查,应用EpiData 3.1软件建立数据库,用SPSS12.0软件进行统计分析.结果 ①1194例的SF-36量表8个领域百分转换得分中,以躯体功能得分最高(86.00±18.48),总健康得分最低(44.37±21.59).②方差分析发现,治疗时间不同的4组患者除社会功能维度外,躯体功能、躯体角色、总健康、生命力、情感角色、心理健康、肌体疼痛7个维度的得分差异有统计学意义(P＜0.01).经两两比较(LSD法)后发现,以上7个维度得分中,抗病毒治疗超过12个月的患者得分明显高于治疗时间少于1个月的患者(新人组)(P＜0.05).结论 抗病毒治疗时间的长短是AIDS患者生命质量的影响因素,治疗超过12个月患者的生命质量比新人组患者明显改善.%Objective To investigate the effect of antiviral therapeutic duration on quality of life among AIDS patients receiving highly active antiretroviral therapy (HAART). Methods A typical sampling method was applied to include a provincial level, two prefecture-level and five county-level hospitals as research sites, and AIDS patients initiating HAART in the 8 hospitals before Apr. 2009 as study subjects. Chinese version SF-36 and basic information questionnaire were used to conduct cross-sectional survey, EpiData 3.1 to create database, and SPSS 12.0 to make statistical analysis. Results ①Among the converted percentile scores for 8 SF-36 scales obtained from the 1194 AIDS patients, physical functioning score was the highest (86.00±18.48), and general health score the lowest (44.37 ±21.59). ②Analysis of variance showed that except social function score, the scores for the other 7 scales, I.e. Physical function, role physical, general health, vitality, role emotional, mental health and bodily pain were significantly different among the 4 groups of patients with different therapeutic duration (P<0.01). LSD method showed that the scores for the 7 scales obtained from the patients receiving HAART for over 12 months were significantly higher than those obtained from the patients receiving HAART for less than 1 month (P<0.05). Conclusions Antiviral therapeutic duration can affect the quality of life among AIDS patients receiving HAART. The quality of life among AIDS patients receiving HAART for over 12 months improves more significantly than that among newly recruited patients.
