病历中患者姓名规范化流程管理

摘要

本文基于病历中患者姓名错误原因及其负面影响的分析，制定患者姓名确认及更正的规范化流程。采用持“身份证”挂号，多环节核查，以及实行开具《患者姓名信息更正证明》的方式。一年后患者姓名错误发生率同比降低57％，相关纠纷减少，取得初步成效。%On the basis of the analysis on the reasons and the negative impacts of the mistakes in the patient’s name in the medical records, the author put forward a normalized process for correcting the patient ’s name. By the methods of registration with identity card, multi -link verification and establishment of “Correct Certification for Mistaken Patient’s Personal Information”, the error rate of mistaken patient’s name reduced 57% on an annual basis, which made the medical disputes resulting from such mistakes decline obviously .
