目的 探讨左卡尼汀对病毒性心肌炎患儿血清肿瘤坏死因子-α(TNF-α)及超氧化物歧化酶(SOD)的影响.方法 将60例病毒性心肌炎患儿随机分为两组各30例,治疗组给予左卡尼汀50 ~ 100 mg· kg-1·d-1,静脉滴注;对照组给予辅酶A100U·d-1+三磷腺苷40 mg·d-1,静脉滴注.疗程均为14 d.采用放射免疫分析法检测患儿治疗前后血清TNF-α、SOD水平.结果 对照组和治疗组TNF-α治疗前分别为(629.2±30.8),(628.6±30.4) ng·L-1,治疗后分别为(604.6±28.9),(346.5±19.7)ng·L-1(P<0.01);SOD水平治疗前分别为(74.9±7.6),(72.5±6.8)U·mL-1,治疗后分别为(78.3±7.8),(150.3±13.1)U·mL-1(P<0.01).结论 左卡尼汀可降低血清TNF-α水平,增加SOD水平,对病毒性心肌炎有良好的治疗作用.%Objective To study the effects of levocamitine on the serum levels of TNF-α and SOD in children with viral myocarditis. Methods Sixty children with viral myocarditis were randomly divided into two groups with 30 cases in each. The treatment group was administrated with levocamitinee at the dose of 50- L00 mg · kg-1·d-1 by inlranvenoos drip. The control group(n = 30) was injected with CoA ( 100 U · d-1) +ATP at the dose of 40mg·d-1 in the same manner. The serum levels of TNF-α and SOD in patients were delected by RIA before and after treatment . Results The serum level of TNF-α in the control group and treatment one before treatment was ( 629.2 ± 30. 8) ng · L-1 and ( 628. 6 ± 30.4) ng·L-1,respectively, while that of TNF-α after being treated was (604.6±28.9) ng·L-1 and (346.5±19.7) ng·L-1 , respectively (P<0. 01 ). The pre-treatment and post-treatment serum level of SOD was (74.9±7.6) U·mL-1 and (72.5±6.8) U · mL-1,(78.3±7.8)U · mL-1 and (150. 3±13. 1) U · mL-1 ,respectively(,P<0.01 ) Conclusion Levocarnitine can reduce, the serum levels of TNF-α and increase SOD,which exerting a good therapeutic effect un viral myocarditis.
