服用奥氮平与利培酮的精神分裂症患者心电图比较分析

摘要

Objective To study the cause of electrocardiogram change of schizophrenia patients with olanzapine and risperidone .Methods 290schizophreniapatientsfromJanuary,2011toNovember,2014inthehospitalwereselected.132casestookolanzapine ( olanzapine group ) , and 158 cases took risperidone ( Risperdal group ).Relevant medical records were recorded and statistical analysis was done.Results Abnormal ST -T and QRS wave group of olanzapine group compared with risperidone group has significantly statistical difference.The mean P-R intervals in patients with olanzapine is (0.14 ±0.07), which is significantly lower than the line average (0.16 ±0.07) (P=0.023);the mean QT intervals in patients with olanzapine is( 0.042 ±0.02), which is significantly higher than that of the baseline mean (0.040 ±0.02) (P=0.003);in the QT eventual interval of the patients , the average is (0.043 ±0.02), which is signifi-cantly higher than that of the baseline mean (0.040 ±0.01) (P=0.015).Conclusion Comparable propensity of olanzapine and risperi-done for induction of electrocardiographic changes demands adequate cautiousness by clinicians , particularly with respect to shortening of QT interval that is mainly noticeable in the olanzapine group.%目的：对服用奥氮平与利培酮精神分裂症患者的心电图进行比较分析。方法选取2011－01～2014－11间在我院治疗的精神分裂症患者290例，其中服用奥氮平的患者132例（奥氮平组），服用利培酮的患者158例（利培酮组）。记录病例资料并做统计学分析。结果奥氮平组和利培酮组患者在ST－T异常和QRS波群方面比较具有统计学差异。奥氮平组患者P－R间期最终均值（0.14±0.07），比基线均值（0.16±0.07）明显降低（P＝0.023）；奥氮平组患者QT间期最终均值（0.042±0.02），比基线均值（0.040±0.02）明显增高（P＝0.003）；利培酮组患者QT间期最终均值（0.043±0.02），比基线均值（0.040±0.01）明显增高（ P＝0.015）。结论奥氮平与利培酮用于精神分裂症的治疗时，会导致患者心电图变化，临床医生对这一影响应足够重视，尤其奥氮平可明显缩短精神分裂症患者QT间期。
