PCT、CRP、WBC联合检测在新生儿感染性肺炎诊断及疗效判断中的价值研究

             

摘要

目的:分析降钙素原( PCT)、C反应蛋白( CRP)、白细胞( WBC)联合检测方法在感染性新生儿肺炎诊断及治疗过程中的应用价值。方法将我院97例新生儿感染性肺炎患儿纳入作为观察组,根据感染性质分为细菌感染组51例和混合感染组(非细菌感染)46例。另选同期97例非感染性疾病新生儿作为对照组。分析两组新生儿出生后体内PCT、CRP、WBC水平的变化对诊断结果的灵敏性及特异性。结果治疗前对照组PCT、CRP、WBC的水平明显低于观察组,差异有统计学意义( P<0.05);治疗一段(72 h)时间后,细菌感染组血清PCT、CRP及WBC水平明显较治疗前降低(P<0.05);混合感染组血清PCT、CRP水平及WBC计数均明显降低(P<0.05);对照组(入院后72 h),WBC计数无明显变化(P>0.05);对照组、细菌感染组、混合感染组的PCT、CRP、WBC联合检测灵敏性分别为79.8%、91.3%、79.3%,特异性为72.4%、86.4%、70.3%。结论 PCT、CRP、WBC联合检测方法在辅助诊断及疗效监测新生儿感染性肺炎的过程中具有重要指导价值,适合在临床中作为其血清学感染指标检测的常规指标模组。%Objective To analyze the value of PCT, CRP, WBC joint detection method in the diagnosis and treatment of neonatal pneumoniainfection.Methods 97casesofneonatalinfectionpneumoniaweretreatedastheobservationgroup,andonthebasisofthein-fection, they were divided into 46 cases with bacterial infection( observation group 1) and 51 cases of mixed infection( observation group 2).Another 97 cases of non infectious diseases were regarded as the control group.The sensitivity and specificity of the diagnosis result caused by PCT, CRP and WBC were analyzed.Results Before the treatment, PCT, CRP, WBC of the control group were significantly lower than observation group (P<0.05);after a period of treatment, in the bacterial infection group, serum PCT levels of CRP and WBC were significantly lower than those before therapy (P<0.05);serum PCT, CRP and WBC levels in mixed infection group were significantly decreased (P<0.05).There wasn′t significant changes of WBC in the control group after 72h admission(P>0.05).The combination sensitivity of PCT, CRP and WBC in the observation group, bacterial infection group and mix group were 79.8%, 91.3% and 79.3%, and the specificity of the two groups were 72.4%, 86.4%and 70.3%.Conclusion PCT, CRP and WBC joint detection method has im-portant guiding value in diagnosis and treatment monitoring of auxiliary infectious neonatal pneumonia, and it is suitable to be the normal in-dex model of serology infection index testing.
