机械通气联合血液灌流在重度有机磷中毒中的疗效及护理观察

摘要

目的:探讨机械通气联合血液灌流在重度有机磷中毒中的疗效及护理对其疗效的影响.方法:选取2007年12月至2011年6月于本院进行治疗的52例重度有机磷中毒患者为研究对象,均采用机械通气联合血液灌流进行治疗,将其根据护理方式分为A组(常规护理组)26例和B组(整体护理干预组)26例,后将两组患者的死亡率、达到阿托品化的时间、阿托品总量、CHE恢复时间、并发症发生率、住院时间及治疗前、治疗后1d、3d、7d的血清心肌酶、CRP、IL-6水平进行统计及比较.结果:B组死亡率及并发症发生率低于A组,达到阿托品化的时间、CHE恢复时间及住院时间均短于A组,阿托品总量小于A组,治疗后1d、3d、7d的血清心肌酶、CRP、IL-6水平均低于A组,P均＜0.05,均有显著性差异.结论:机械通气联合血液灌流在重度有机磷中毒中的疗效较佳,配合整体护理干预可以显著改善综合疗效.%Objective: To study the effect of mechanical ventilation combined with hemoperfusion in severe organophosphorus poisoning and the influence of nursing for the effect. Method :52 patients with severe organophosphorus poisoning in our hospital from December 2007 to June 2011 were selected as research object ,and they all were treated with mechanical ventilation combined with hemoperfusion. All the patients were divided into group A( routine nursing group )26 cases and group B( holistic nursing intervention group )26 cases , then the mortality rate , time of reaching atropinization , amount of atropine , recovery time of CHE , incidence of complications ,hospitalization time and serum myocardial enzyme, CRP, IL-6 before and after the treatment at first,third and seventh day of two groups were analyzed and compared. Result:The mortality rateand incidence of complications of group B were lower than those of group A, time of reaching atropinization , recovery time of CHE and hospitalization time were shorter than those of group A, amount of atropine was smaller than that of group A,serum myocardial enzyme,CRP,IL-6 after the treatment at first,third and seventh dy were lower than those of group A, all P<0. 05 , there were significant differences. Conclusion:The effect of mechanical ventilation combined with hemoperfusion in severe organophosphorus poisoning is better, and it combined with holistic nursing intervention can significantly improve the comprehensive therapeutic effect.
