目的:探讨胸腔镜与传统手术对食管癌的临床疗效与术后体液及细胞免疫功能的影响效果.方法:收集2014年1月至2017年3月入院的80例食管癌患者随机分为两组,胸腔镜组患者给予胸腔镜手术,传统组患者则给予传统手术,比较两组患者围手术期指标、临床近期疗效、并发症、体液及细胞免疫功能与生活质量.结果:胸腔镜组患者切口长度、术中出血量与总体并发症率均显著性低于传统组,淋巴结清扫个数显著性高于传统组;术后7d IgA、IgG、IgM、CD3+、CD4+、CD4+/CD8+与NK水平组间比较均显著性高于传统组,CRP水平显著性低于传统组;术后12周躯体功能评分组间比较显著性高于传统组,恶心与呕吐、疼痛与呼吸困难评分显著性低于传统组,差异有统计学意义(P<0.01).结论:相比较传统手术,胸腔镜手术的临床效果更为显著,对免疫功能影响较小,具有借鉴意义.%Objective:To investigate the effect of thoracoscopy and traditional surgery on the clinical ef-ficacy of esophageal carcinoma and postoperative humoral and cellular immune function. Methods:80 patients with esophageal cancer who were enrolled from January 2014 to March 2017 were randomly divided into two groups. Thoracoscopic surgery was performed in patients undergoing thoracoscopic surgery. The patients in the traditional group were given traditional surgery. The clinical short-term efficacy, complications, body fluids and cellular immune function and quality of life of two groups were compared. Results: The incision length, intraoperative blood loss and overall complication rate were significantly lower in the thoracoscopic group than in the traditional group. The number of lymph node dissection was significantly higher than that of the conven-tional group. After 7 days, the levels of IgG, IgM, CD3 +, CD4 + / CD8 +and NK were significantly higher than the traditional group, CRP level were significantly lower than the traditional group; 12 weeks later the physical function scores were significantly higher than the traditional group. nausea and vomiting, pain and dif-ficulty breathing was significantly lower than the traditional group, the difference was statistically significant (P <0.01). Conclusion: Compared with traditional surgery, the clinical effect of thoracoscopic surgery is more significant, which has little effect on immune function and has reference significance.
