Objective:To investigate the curative effect of vertebroplasty combined with cement-reinforced pedicle screw fixation in the treatment of severe osteoporotic spondylolisthesis(OVCF).Methods: 80 patients with severe OVCF,who were enrolled from May 2014 to May 2016,were randomly divided into two groups.The patients in the control group received conservative treatment.The patients in the experimental group were treated with vertebroplasty combined with bone cement fixation,comparison of the two groups of patients with fracture-related parameters,hospital economics benefits,fracture rate,quality of life,imaging grade and satisfaction.Results: Compared with the control group,the VAS score,the height loss rate of the vertebral body,the score of the analgesic drug used in the experimental group were significantly higher than those in the control group(P <0.05).The bone mineral density(BMD),the cost of hospitalization and drug cost were significantly higher than those of the control group,the cost of rehabilitation,the cost of escort,the cost of rehabilitation,the cost of rehabilitation,the time of hospitalization and the rate of recurrence were all significantly lower than those of the control group(P <0.05).The imaging grade was significantly better than that of the control group(P <0.05).There was significant difference between the two groups(P <0.05).Conclusion: The treatment of severe OVCF with vertebroplasty combined with bone cement for pedicle screw fixation is significant and has reference significance.%目的:探讨椎体成形术联合骨水泥强化椎弓根钉内固定治疗重度骨质疏松性脊柱压缩骨折(OVCF)的疗效.方法:收集2014年5月至2016年5月入院的80例重度OVCF患者随机分为两组,对照组患者给予保守治疗,实验组患者则采用椎体成形术联合骨水泥强化椎弓根钉内固定术,比较两组患者骨折相关参数、住院经济学效益、再骨折率、生活质量、影像学分级与满意度.结果:实验组患者治疗后12个月骨密度值、各维度生活质量评分与健康变化总分组间比较均显著高于对照组,VAS评分、椎体高度丢失率、镇痛药使用评分与脊柱活动能力评分组间比较均显著低于对照组;住院费用与药品费用均显著高于对照组,康复费用、陪护费用、康复期药品费用、康复期理疗费用、住院时间与再骨折率均显著低于对照组;满意度显著优于对照组,存在显著差异(P<0.05);影像学分级明显优于对照组,存在明显差异(P<0.05).结论:椎体成形术联合骨水泥强化椎弓根钉内固定治疗重度OVCF的疗效显著,具有借鉴意义.
