Objective To observe the clinical effects of Wu's manipulation,herbal hot compress and functional training on scapulohumeral periarthritis of adhesion stage.Methods 69 patients with scapulohumeral periarthritis of adhesion stage were randomly divided into the two groups.35 cases in the treatment group were treated by Wu's manipulation,herbal hot compress and functional training,34 cases in the control group were treated by Diclofenac sodium sustained-release tablets for oral and Diclofenac diethylamine emulsion for external application.The treatment course was two weeks,and the curative effects were evaluated.The improvements of shoulder pain and tenderness,the range of motion of shoulder anteflexion,rear protraction,abduction,adduction,internal rotation and extorsion were observed at 1st,2nd,4th and 12th week in two groups.Results The score of shoulder pain visual analogue scale (VAS) after treatment one week was reduced in the treatment group,and which after treatment two week was reduced in the control group (P ＜0.05).The scores of shoulder pain VAS at 1st,2nd,4th and 12th week in the treatment group were lower than those in the control group at the same period (P ＜ 0.05,).The shoulder pain were significantly improved after treatment 1 st,2nd,4th and 12th week in two groups (P ＜ 0.05).There was no statistical difference on the shoulder pain between the two groups after treatment one week (P ＞ 0.05).There were statistical differences on the shoulder pain between the two groups after treatment two,four and twelve weeks,and the improvements in the treatment was superior to that in the control group (P ＜ 0.05).The range of motion of shoulder anteflexion,rear protraction,abduction,adduction,internal rotation and extorsion was significantly improved in the treatment group after treatment one week (P ＜ 0.05).The range of motion of shoulder anteflexion and internal rotation was significantly improved after treatment 1 st week in the control group (P ＜ 0.05),and the range of motion of rear protraction,abduction,adduction and extorsion improved after treatment 2nd week (P ＜ 0.05).There was no statistical difference between the two groups except the range of motion of extorsion after treatment 1 st in two groups (P ＞ 0.05),with statistical differences on each range of motion at the other times (P ＜ 0.05).Conclusion Wu's manipulation,herbal hot compress and functional training,on scapulohumeral periarthritis of adhesion stage have a rapid onset of effect and long duration of action,not easy to repeat.%目的 观察吴氏手法配合中药热敷、功能锻炼治疗肩关节周围炎(以下简称肩周炎)粘连期的临床疗效.方法 将69例肩周炎粘连期患者随机分为2组.治疗组35例予吴氏手法配合中药热敷、功能锻炼治疗;对照组34例采用双氯芬酸钠缓释片口服加双氯芬酸二乙胺乳胶剂外用治疗.2组均2周为1个疗程,1个疗程后统计临床疗效,并观察比较2组治疗后第1、2、4、12周肩关节疼痛、压痛改善情况及肩关节前屈、后伸、外展、内收、内旋、外旋6个方向活动度改善情况.结果 治疗组治疗1周、对照组治疗2周开始,肩关节疼痛视觉模拟评分(VAS)较本组治疗前明显下降(P＜0.05);2组组间比较显示,治疗组治疗1、2、4、12周疼痛VSA均低于对照组同期(P＜0.05).2组治疗后1、2、4、12周肩部压痛均较本组治疗前明显改善(P＜0.05);2组组间比较显示,治疗后1周2组肩部压痛比较差异无统计学意义(P＞0.05),治疗后2、4、12周治疗组肩部压痛改善优于对照组(P＜0.05).治疗后2组肩关节前屈、后伸、外展、内收、内旋、外旋活动度较本组治疗前明显改善,治疗组均在治疗后1周改善(P＜0.05);对照组前屈、内旋在治疗后第1周出现(P＜0.05),后伸、外展、内收、外旋在治疗后第2周出现(P＜0.05);治疗后组间比较显示,除第1周2组间外旋活动度比较差异无统计学意义(P＞0.05)外,其余同时间点各方向活动度比较差异均有统计学意义(P＜0.05),治疗组改善情况优于对照.结论 吴氏手法配合中药热敷、功能锻炼治疗肩周炎粘连期起效快,疗效持久,不易反复.
