颅脑外伤引起的迟发性脑出血与凝血功能障碍的临床研究

             

摘要

Objective To establish the limit value of blood coagulation indexes in patients with traumatic brain injury, and to provide reference for early identification of delayed cerebral hemorrhage. Methods Selection in 2010 to 2015 to March 3 monthly in-come of craniocerebral injury patients in our hospital a total of 267 cases, which is divided into primary cerebral hemorrhage group ( group A, n = 102 cases) , delayed intracerebral hemorrhage group ( group B, n = 86 cases) and no in cerebral hemorrhage group (group C, n = 79) were used to detect the admission, 2h, 4h, 6h and 12h time point of prothrombin time (PT), fibrin original ( FG) and D dimer ( D-D) level changes. Results In group A and group B of Pt, FG and D-dimer levels and peak are higher than that of group C, differences were statistically significant (P<0. 05), B group the index peak time delay, difference in the level of no statistical significance (P>0. 05). Conclusion The levels of PT, Fg and D-D can be detected in the early stage of brain injury.%目的:建立颅脑外伤患者继发迟发性脑出血的凝血指标界限值,为指导临床早期识别迟发性脑出血提供参考依据。方法选择2010年3月至2015年3月入我院的颅脑外伤患者共267例,将其分为初发脑出血组( A组,n=102例)、迟发性脑出血组(B组,n=86例)和无脑出血组(C组,n=79例),分别检测入院2h、4h、6h和12h时间点的凝血酶原时间(PT)、纤维蛋白原( Fg)和D-二聚体( D-D)水平的变化情况。结果 A组和B组的PT、Fg和D-D水平和峰值均比C组高,差异均有统计学意义(P<0.05),B组上述指标峰值时间延迟,差异无统计学意义(P>0.05)。结论检测PT、Fg和D-D的水平和峰值时间可以早期预测颅脑外伤患者继发迟发性脑出血,有较大的临床推广价值。
