不同手术方法对白内障患者眼压的影响

摘要

Objective To investigate the effect of phacoemulsification surgery, small incision non-phaco-emulsification surgery on the intraocular pressure (IOP) in patients with cataract. Methods Three hundred and twenty patients with cataract in our hospital were randomly divided into group A and group B. The patients in group A were treated by phacoemulsification surgery, while those in Group B patients with small incision non-phacoemulsification surgery. The perioperative intraocular pressure (IOP), visual acuity and complications were compared between the two groups. Results The IOP in both groups at postoperative 2 h, 4 h, 10 h, 24 h were significantly higher than those before treatment (P<0.05). The IOP at postoperative 48 h showed no statistically significant difference with that before treatment (P>0.05). The IOP at postoperative 2 h, 4 h, 10 h, 24 h were significantly higher in group B than group A (P< 0.05). The visual recovery was significantly higher in group A than group B one week after treatment (P<0.05), and showed no statistically significant difference after 3 months (P>0.05). No statistically significant difference were found in intraoperative and postoperative complications between the two groups (P>0.05). Conclusion Compared with phacoemulsification surgery, small incision non-phacoemulsification surgery is more likely to lead to the increase of intraocular pressure in patients with cataract.%目的 探讨超声乳化吸除术和小切口非超声乳化吸除术治疗对白内障患者眼压的影响.方法 将我院收治的320例白内障患者按手术方式不同分为A组和B组,A组采用超声乳化吸除术治疗,B组采用小切口非超声乳化吸除术治疗,观察两组患者围手术期眼压、视力恢复情况以及并发症情况.结果 两组患者术后2h、4h、10h、24 h眼压均高于术前(P＜0.05)；而术后48 h的眼压与术前比较,差异无统计学意义(P＞0.05)；两组患者眼压术后2h、4h、10h、24h时间点比较,B组患者眼压高于A组,差异有统计学意义(P＜0.05);A组患者术后1周内的视力恢复情况明显好于B组(P＜0.05),术后3个月的视力两组比较差异无统计学意义(P＞0.05)；两组患者术中以及术后出现并发症的例数差异无统计学意义(P＞0.05).结论 小切口非超声乳化术较超声乳化吸除术更容易导致白内障患者围手术期眼压升高.
