绝经后妇女子宫内膜息肉发病相关因素分析

             

摘要

目的:探讨绝经后妇女子宫内膜息肉(EP)发病的相关危险因素。方法选取2013年3月至2015年12月期间在我院妇产科经超声、宫腔镜以及病理检查的绝经妇女296例,根据病理结果进行分组,其中103例绝经后子宫内膜正常者为正常对照组,另193例绝经后EP患者为EP组,记录并比较分析两组受检者的临床资料,通过单因素以及Logistic多因素分析EP发病危险因素。结果绝经后EP高发于40~60岁年龄段,单因素分析显示, BMI、肥胖、宫颈息肉、子宫慢性内膜炎、子宫腺肌症、高血压以及激素治疗史均为EP发病相关因素(P<0.05),孕次及产次为EP的保护性因素(P<0.05);Logistic多因素分析显示,肥胖、宫颈息肉、子宫慢性内膜炎、子宫腺肌症及激素治疗均为绝经后EP发病独立危险因素(OR=2.602、2.733、1.871、2.467、3.689,P<0.05)。结论肥胖、宫颈息肉、子宫慢性内膜炎、子宫腺肌症及激素治疗均为绝经后EP发病独立危险因素,且高血压患者也具有较高的EP发生风险,孕产次可能是EP发病的保护因素。%Objective To study the associated risk factors of endometrial polyps (EP) in postmenopausal wom-en. Methods A total of 296 patients from Department of Gynaecology and Obstetrics in our hospital who were checked by ultrasound, hysteroscopy and pathology during March 2013 to December 2015 were grouped according to the patholo-gy results: 103 cases of normal postmenopausal endometrium were regarded as the normal control group, and another 193 cases of EP patients belonged to the EP group. The clinical data of the two groups were recorded and compared. The risk factors of EP were analyzed by univariate and logistic multivariate analysis. Results Postmenopausal EP occurred highly at the age of 40 to 60. Univariate analysis showed that body mass index (BMI), obesity, cervical polyps, chronic endometritis, adenomyosis, hypertension, and history of hormone therapy were the related factors of EP (P<0.05). The number of pregnancies and the number of production were the protective factor of EP (P<0.05). Logistic multivariate analysis showed that obesity, cervical polyps, chronic endometritis, adenomyosis, postmenopausal hormone therapy were the independent risk factors of EP (OR=2.602, 2.733, 1.871, 2.467, 3.689, P<0.05). Conclusion Obesity, cervical polyps, chronic endometritis, adenomyosis and postmenopausal hormone therapy were independent risk factors of EP af-ter the onset of menopause, and high blood pressure also presents a higher risk of occurrence of EP. The number of preg-nancies and the number of production may be the protective factors of EP.
