左乙拉西坦对癫痫患者血清细胞因子及认知状态相关指标的影响

             

摘要

目的 观察左乙拉西坦对癫痫患者血清细胞因子及认知状态相关指标的影响.方法 选取佛山市第一人民医院2015年7月至2016年4月收治的40例癫痫患者为研究对象,将其根据治疗方法的不同分为对照组(丙戊酸钠组)20例和观察组(丙戊酸钠加左乙拉西坦组)20例,比较两组患者的临床疗效、不同时间的血清细胞因子及认知状态相关指标.结果 观察组的临床总有效率为95.00%,高于对照组的70.00%,差异有统计学意义(P>0.05),治疗前两组患者的血清细胞因子及认知状态相关指标比较差异均无统计学意义(P>0.05);治疗后1个月、3个月及6个月观察组的血清脑源性神经营养因子(BDNF)水平分别为(30.08±3.25)ng/mL、(33.13±3.42)ng/mL及(36.76±3.78)ng/mL,高于对照组及本组治疗前,差异有统计学意义(P<0.05),血清细胞因子白介素6(IL-6)、白介素8(IL-8)、肿瘤坏死因子α(TNF-α)、白介素2(IL-2)、白介素4(IL-4)、白介素10(IL-10)水平及其他认知状态相关指标神经胶质纤维酸性蛋白(GFAP)、髓鞘碱性蛋白(MBP)水平均低于对照组及本组治疗前,差异有统计学意义(P<0.05).结论 左乙拉西坦对癫痫患者血清细胞因子及认知状态相关指标的影响较好,临床应用价值较高.%Objective To observe the influence of levetiracetam on the serum cytokines and cognitive state relat-ed indexes of patients with epilepsy. Methods A total of 40 patients with epilepsy, who admitted to our hospital from July 2015 to April 2016, were selected and divided into the control group (sodium valproate group) and the observation group (sodium valproate and levetiracetam group) according to the difference of treatment methods, with 20 cases in each group. The clinical effect, serum cytokines and cognitive state related indexes of two groups at different time before and after the treatment were compared. Results The total effective rate of the observation group was 95.00%, which was significantly higher than 70.00%of the control group (P<0.05). There was no significant difference in the serum cytokines and cognitive state related indexes of two groups before the treatment (P>0.05); while the serum brain derived neurotrophic factor (BDNF) level of the observation group at the first, third and sixth month after the treatment were (30.08 ± 3.25) ng/mL, (33.13 ± 3.42) ng/mL and (36.76 ± 3.78) ng/mL, respectively, which were all significantly higher than those of the control group and those before the treatment (P<0.05), and serum cytokines interleukin 6 (IL-6), interleukin 8 (IL-8), tumor necro-sis factorα(TNF-α), interleukin 2 (IL-2), interleukin 4 (IL-4), interleukin 10 (IL-10) and other cognitive state related in-dexes including glial fibrillary acidic protein (GFAP), myelin basic protein (MBP) were all significantly lower than those of the control group and those before the treatment (P<0.05). Conclusion Levetiracetam has a better influence on the se-rum cytokines and cognitive state related indexes of patients with epilepsy, which has a higher clinical application value.
