无创正压通气治疗OSAHS的疗效及其对患者血清hs-CRP、HCY及NO水平的影响

             

摘要

Objective To observe the efficacy of noninvasive positive pressure ventilation (NPPV) in the treat-ment of obstructive sleep apnea hypopnea syndrome (OSAHS) and its affection on patients' serum hypersensitive C-reac-tive protein (hs-CRP), homocysteine (Hcy) and nitric oxide (NO) levels. Methods From March 2014 to March 2016, 246 cases of OSAHS patients who admitted to Department of Internal Medicine of Xi'an Medical University were se-lected and randomly classified into two groups using random number table method, with 123 cases in each group. The control group was treated with normal therapy, and the observation group was treated with NPPV based on the treat-ment of the control group. The treatment course was 4 weeks. The changes of polysomnography monitoring related in-dicators, quality of life and serum hs-CRP, Hcy and NO levels before and after treatment in two groups were com-pared. Results After the treatment, apnea-hypopnea index, average SaO2 level, lowest SaO2 level, hs-CRP level, Hcy level and NO level in the observation group were (7.8 ± 2.2) times/h, (0.93 ± 0.03)%, (0.88 ± 0.08)%, (0.89 ± 0.56) mg/L,(17.86±5.21)μmol/L, (20.16±5.19)μmol/L, respectively, and (22.4±5.7) times/h, (0.89±0.04)%, (0.81±0.07)%, (1.35± 0.68) mg/L, (21.36±6.51)μmol/L, (15.69±4.36)μmol/L in the control group, respectively. Compared with pretreatment, apnea-hypopnea index was significantly deceased, and average SaO2 and lowest SaO2 levels were increased in both groups after the treatment, and significant differences were observed (P<0.05). Moreover, the improvement of apnea hy-popnea index, average SaO2 and lowest SaO2 levels in the observation group were significantly better than those in the control group (P<0.05). After the treatment, quality of life indicators (physical function, psychological function, social function, material life status) scores in the observation group were significantly better than those in the control group (P<0.05). Conclusion Noninvasive positive pressure ventilation can improve sleep, enhance life quantity, degrade serum hs-CRP, Hcy, and increase serum NO for OSAHS patients, which is worthy of clinical application.%目的 观察无创正压通气治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床效果及其对患者血清超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)及一氧化氮(NO)水平的影响.方法 选取2014年3月至2016年3月西安医学院内科收治的246例OSAHS患者,按随机数表法分为对照组和观察组,每组123例.对照组采用常规治疗,观察组在对照组基础上采用无创正压通气治疗,治疗4周,比较治疗前后两组患者的多导睡眠监测相关指标、生活质量以及血清hs-CRP、Hcy及NO的水平.结果 观察组患者治疗后的呼吸暂停低通气指数、平均SaO2、最低SaO2、hs-CRP、Hcy、NO分别为(7.8±2.2)次/h、(0.93±0.03)%、(0.88±0.08)%、(0.89±0.56)mg/L、(17.86±5.21)μmol/L、(20.16±5.19)μmol/L,而对照组分别为(22.4±5.7)次/h、(0.89±0.04)%、(0.81±0.07)%、(1.35±0.68)mg/L、(21.36±6.51)μmol/L、(15.69±4.36)μmol/L,两组患者治疗后的呼吸暂停低通气指数下降,平均SaO2、最低SaO2水平升高,与治疗前比较差异均有统计学意义(P<0.05),且观察组呼吸暂停低通气指数、平均SaO2、最低SaO2的改善情况优于对照组,差异均有统计学意义(P<0.05);观察组患者治疗后的生活质量指标(躯体功能、心理功能、社会功能、物质生活状态)评分均明显优于同期对照组,差异均有统计学意义(P<0.05).结论 无创正压通气能够明显改善OSAHS患者的睡眠情况,提高生活质量,降低血清hs-CRP、Hcy水平,升高血清NO水平,值得临床推广应用.
